Objective: 1.To study the correlation between Gleason grade and Ki-67 protein expression in prostate cancer.2.To study the correlation between ceus parameters and Ki-67 protein expression in prostate cancer.3.ROC curve was used to evaluate the accuracy of differentiating the expression of Ki-67 protein by contrast-enhanced ultrasound parameters in prostate cancer.Methods:Select ultrasonic imaging abnormalities were retrospectively analyzed and targeted by transrectal prostate biopsy and systematic biopsy was diagnosed with prostate cancer patients,contrast-enhanced ultrasound dynamic image in selected patients were analyzed,and the observed image characteristics,selection of imaging mode for the nonuniformity,and diffuse of nodular enhancement of images,identify abnormal imaging area,quantitative analysis of the abnormal imaging area with QLAB software for imaging parameters.The pathological Gleason score was determined and the immunohistochemical expression of Ki-67 protein was detected.According to the expression intensity of Ki-67 protein,it was divided into negative group,low expression group and high expression group.The correlation between Gleason grade and Ki-67 protein expression in prostate cancer was analyzed.The correlation between CEUS parameters and Ki-67 protein expression with different intensities was analyzed.ROC curve was used to evaluate the accuracy of ceus parameters in differentiating the expression of Ki-67 protein.Results: According to the inclusion and exclusion criteria,a total of 113 prostate cancer patients were included,including 41 in the Ki-67 protein-negative group,51 in the low-protein group and 21 in the high-protein group.According to the results of statistical analysis,Gleason grade of prostate cancer showed statistically significant differences in Ki-67 protein expression among different groups(P<0.05),with a significant positive correlation(r=0.742,P<0.001).Contrast-enhanced ultrasound(CEUS)parameters RT,PI,AUC,WIS and TTP showed statistically significant differences in different Ki-67 protein expression groups(P<0.05).Contrast parameters RT and TTP were negatively correlated with Ki-67 protein expression(r=-0.335,P<0.001,r=-0.355,P<0.001).Contrast parameters PI,AUC and WIS were positively correlated with Ki-67 protein expression(r=0.379,P<0.001,r=0.401,P<0.001,r=0.361,P<0.001).The contrast parameters HT and MTT in different Ki-67 protein expression groups showed no significant difference(P>0.05),and there was no correlation.According to the ROC curve analysis results of Ki-67 low protein expression,the ROC area under the curve(AUC)of Ki-67 low protein expression was 0.671,0.610,0.523,0.622,0.502,0.629,and 0.693,respectively,with contrast-enhanced ultrasound parameters RT,PI,MTT,AUC,HT,WIS,and TTP.According to distinguish Ki-67 protein expression of ROC curve analysis results show that the ultrasonic imaging parameters of RT,PI,determined by MTT,AUC,HT,WIS,TTP to distinguish the area under the curve of Ki-67 high protein expression were 0.732,0.808,0.526,0.812,0.631,0.776,0.742,the imaging parameters PI,AUC,WIS to distinguish between protein high expression of Ki-67 had a higher accuracy,AUC diagnostic imaging parameters sensitivity is 80.95%,the specific degree of PI is 85.37%.The sensitivity and specificity of WIS were 76.19% and 82.93%,and the diagnostic thresholds of AUC,PI and WIS were >551.23 d Bs,>10.07 d B and >1.15 d B/s,respectively.Conclusions: Prostate ultrasound imaging parameters of RT,PI,AUC,WIS,TTP associated with expression of Ki-67,including imaging parameter AUC,PI,WIS to distinguish Ki-67 high protein expression has a higher accuracy,these meaningful parameters for noninvasive assessment of the degree of tumor cell proliferation and tumor of prostate cancer angiogenesis provides certain imaging basis for evaluation of tumor malignant degree and prognosis is of certain value.
