OBJECTIVE: The occupational environment of public hospitals is closely related to the satisfaction of medical staff,and directly affects the quality of medical services,work efficiency,and the stability of medical team.The purpose of this study is to understand the current situation of public hospitals' occupational environment,analyze existing problems,and explore the effects of interventions.To provide scientific basis on policy formulation,hospital management,and personal development,so as to improve the occupational environment of public hospitals,maintain the stability of medical staff,ensure the quality of medical services,and promote the sustainable development of medical services.METHODS: On the basis of clarifying relevant concepts,this study uses literature research method,expert consultation method,questionnaire survey method and other methods to investigate the current situation of occupational environment in Shanghai public hospitals,and analyze the impact of different occupational environmental factors on medical staff satisfaction through data processing analysis.Based on whether or not to participate in the activities of a tertiary hospital in Shanghai,the personnel were stratified,and the effect of community activities on the mental health of medical staff was evaluated through questionnaire survey.RESULTS: Through the self-reported public hospital occupational environment questionnaire to investigate the current situation of medical staff in a tertiary public hospital in Shanghai,it is found that the current public hospital occupational environment presents different characteristics at the social,organizational and individual perspective.At the social perspective,medical personnel are concerned about the industry policy environment,but the current health legal system in China is still not sound enough.The media and the medical sector lack positive interaction,the doctor-patient relationship is tense,the medical staff has high occupational risk,and the self-identity is low.At the organizational perspective,medical staff keep working overtime,suffering excessive work pressure,occupational injuries,and poor occupational safety.At the same time,medical staff are not satisfied with the physical working environment,rarely participate in organization management,and they are not fully felt the organizational authorization,but the atmosphere of teamwork is harmonious.At the individual perspective,the medical staff are poor in physical and mental health and have difficulties in balancing work and family.Different occupational environmental factors have different degrees of influence on the satisfaction of medical staff.The rankings from high to low are physical and mental health,physical working environment,occupational risk,organizational system,doctor-patient relationship,team atmosphere factor,work intensity factor and policies and regulations factor.Practice research shows that community activities in public hospitals,which have a good effect on optimizing the professional environment at the organizational perspective,relieving the pressure and improving the physical and mental health of medical staff.Those who participate in community activities are better than those who do not participate in all factors.Participating in different types of associations can improve different factors.The more participating activities,the more beneficial to improving various physical and mental health problems,the length of participation in the community,and the frequency of participation.It has no promoting effect on improving physical and mental health.CONCLUSIONS: The results of the comparative study on the occupational environment status of hospitals at home and abroad show that the current foreign occupational environment related research theories are rich,the research scales are mature,and the research is more refined.Chinese scholars have different understandings of the occupational environment connotation and definition,and the research is limited to nursing.The professional organization environment of personnel lacks a macro analysis of the social environment.The study on the occupational environment status of public hospitals in a tertiary hospital in Shanghai shows that eight factors including social,organizational and individual factors will affect the occupational environment of public hospitals.The most relevant is physical and mental health factors.Since the change of the social environment requires top-level design and is influenced by many factors,it cannot be changed in a short period of time.Therefore,for hospital administrators,it is better to intervene at the organizational level and individual level for improving the professional environment of public hospitals.The study found that using EAP,group dynamics,interpersonal interaction analysis theory and other principles,through the community activities organized in the hospital,is conducive to optimizing the internal organization environment of public hospitals,alleviating the work pressure of medical staff,improving the physical and mental health of medical staff,the more involved in community activities More and more favorable.In view of the eight factors of social,organizational and individual levels in the public hospital's occupational environment,this study proposes to intervene from these three levels in order to comprehensively and effectively optimize the occupational environment of public hospitals.
