Health-promoting Lifestyle And The Influence Factors Among Medical Staff Of Health-promoting Hospitals In Chongqing

Objective: The purpose of this study is to assess the health promoting lifestyle of medical staff of health-promoting hospital in Chongqing and to explore the influence factors.This study is expected to provide a reference for improving the health status and promoting a healthy lifestyle among medical staff.Methods: In this observational study,improbability convenient sampling was used to investigate the registered medical staff of 48 healthpromoting hospitals in 8 districts and counties of Chongqing,and finally 2,773 medical staff were included.Investigation time was from April to June 2019.We used the health-promoting lifestyle profile(HPLP-II)to assess individual health promoting lifestyle level,which was developed by Walker et al.and sinicized by Yan Miaofen et al.This profile including 6 dimensions and 52 items: spiritual growth(9 items),health responsibility(9 items),physical activity(8 items),nutrition(9 items),interpersonal relationship(9 items)and stress management(8 items).Each item was scored on the Likert 4-level scale,with 1 to 4 points from "never","sometimes","often" to "always",and the total score ranged from 52 to 208 points.The scores were divided into four grades: poor,average,good and excellent,with corresponding scores ranging from 52 to 89,90 to 126,127 to 163 and 164 to 208 respectively.The formal survey was conducted by the combination of field survey and questionnaire star survey.Mann-Whitney U test,KruskalWallis H test,Spearman correlation and multiple linear regression were used to analyze the data.Results: The average total score of health promoting lifestyles of medical staff was 123.71±22.70,with the highest score in interpersonal relations(23.19±4.11),followed by nutrition(22.37±4.22),spiritual growth(21.85±4.90),health responsibility(21.12±4.40),stress management(18.51±3.94)and physical activity(16.66±4.66).The average score of 52 items in health-promoting lifestyle for medical staff was 2.38 ± 0.44,and the standardized score was 59.47.The results of univariate analysis showed that the score of healthpromoting lifestyles exhibited statistically significant differences(P<0.05)among different medical staff in different age groups,working years,personality characteristics,hospital grade,occupation,professional title,night shift,monthly average income,health promotion courses or training,discomfort symptoms,chronic diseases,smoking and drinking.The level of self-perceived work stress was negatively correlated with the total score of health-promoting lifestyle(r=-0.246,P < 0.01),while the level of self-perceived health status was positively correlated with the total score of health-promoting lifestyle(r=0.400,P < 0.01).The results of multivariate analysis showed that there were 13 statistically significant factors entered the equation were self-perceived health status,health promotion courses or training,personality characteristics,hospital grade,working years,occupation,professional title,night shift,monthly average income,smoking,self-perceived work stress and discomfort symptoms,and explained 26.5% differences of independent variables.Conclusions: The self-reported health status of medical staff in health-promoting hospital in Chongqing was not good.The level of healthpromoting lifestyle of the health-promoting hospitals' medical staff in Chongqing was medium.Among the six dimensions of health-promoting lifestyle,the interpersonal relations scored the highest,followed by nutrition,spiritual growth,health responsibility,stress management,and physical activity was the worst.The factors affecting the health-promoting lifestyle of medical staff included self-perceived health status,health promotion courses or training personality characteristics,hospital grade,monthly average income,professional title,working years,night shift,smoking,occupation,discomfort symptoms and self-perceived work stress.
