Objective:To investigate the relationship of patients with type 2 diabetes mellitus(T2DM)and squamous cell carcinoma of the buccal mucosa and to identify the impact of T2DM on complications and prognosis in squamous cell carcinoma of the buccal mucosa.Methods:107 patients with squamous cell carcinoma of the buccal mucosa were retrospectively analyzed.19 cases with T2DM and 88 without T2DM were included in T2DM and Non-T2DM group respectively.The patients'age,sex,fasting blood glucose,clinical stage,pathological grade,neck dissection surgery rate metastatic lymph noderate,tumor recurrence rate,postoperative complications of the 2 group and Ki-67 antigen positive rate were compared and analyzed.Results:Total of 107 patients,1.14:1(57/50)in male and female.T2DM group1.375:1(11/8)in male and female,Non-T2DM group 1.09:1(46/42);P=0.656,and the difference between the two groups was not statistically significant.The ratio of early clinical stage(Grade I+Grade II)and late clinical stage(GradeIII+GradeIV)was of 1:3.14(7/22)in T2DM group,and 7.8:1(78:10)in the Non-T2DM group,the difference was statistically significant(?~2=22.59,P<0.001),the difference is statistically significant.T2DM group pathologic stage in well-differentiated squamous carcinoma,moderately differentiated squamous carcinoma,the poorly differentiated squamous carcinoma were 52.6%(10/19),26.3%(5/19),21.1%(4/19);Non?pathological grading in diabetes group in well-differentiated squamous carcinoma,oderately differentiated squamous carcinoma,the poorly differentiated squamous carcinoma were 62.35(55/88),19.3%(17/88),18.1%(16/88),?~2=0.79,P=0.673,there was statistically significant difference.According to the pathological results and postoperative follow-up visit,the rate of cervical lymphadenectomy of DM group and Non-DM group were 89.47%(17/19)and 54.55%(48/88),respectively,the difference was statistically significant(?~2=7.99,P=0.005).Lymph node metastasis rates of DM group and Non-DM group were78.95%(15/19)and 18.2%(16/88),with statistically significant differences(?~2=28.04,P<0.001).The recurrence rates of DM group and Non-DM group were 47.37%(9/19)and 27.3%(24/88),The difference was not statistically significant(?~2=2.96,P=0.085).According to the results,DM group Ki-67 antigen expression in patients with low 8 cases(42.1%),Ki-67 antigen expression in patients with high 11 cases(57.9%);Non-DM group Ki-67 antigen of 59 patients with lower expression(67.1%),29 patients with high Ki-67 antigen expression(32.9%),?~2=7.34,P<0.01,the difference between the two groups have statistical significance.There were 15.8%(3/19)and 6.0%(5/88,4.5%)with postoperative complications in the DM group and Non-DM group respectively,the difference was statistically significant(?~2=3.92,P?0.05)Conclusions:T2DM may be a risk factor for squamous cell carcinoma of the buccal mucosa.T2DM may positively correlate with late clinical stage,high recurrence rate and poor prognosis of squamous cell carcinoma of the buccal mucosa.Blood sugar of patients diagnosed with squamous cell carcinoma of the buccal mucosa and T2DM should be strictly controled in preoperative period,intraoperativeperiod and postoperativeperiod,which will reduce the adverse effects of diabetes on the prognosis of patients.
