Meta-analysis On The Association Of TPOAb And TGAb With Chronic Urticaria

Objective: To systematically evaluate relevant clinical data at home and abroad by means of meta-analysis,aiming to determine whether thyroid peroxidase antibody(TPOAb)and thyroglobulin antibody(TGAb)are correlated with the incidence of chronic urticaria,and to provide reference for the etiology and treatment of chronic urticaria.Methods:Seven databases including PubMed,Cochrane,Embase,CNKI,CBM,VIP and wanfang were retrieved.Case-control studies on the correlation between TPOAb or TGAb and the incidence of chronic urticaria were collected from the database until August 2019,and the Newcastle-Ottawa scale(NOS)of the caseControl study was used for quality evaluation.The eligible studies were systematically evaluated by RevMan5.3 software and Stata13.0 software.Results:(1)A total of 28 case-control studies were included,there were4318 cases of chronic urticaria,including 361 cases of TPOAb(+)and 394 cases of TGAb(+).And in the healthy control group about 4736 cases,including 208 cases of TPOAb(+)and 58 cases of TGAb(+).(2)There were 18 case-control studies satisfying the correlation between TPOAb and chronic urticaria,and the results of meta-analysis showed that the positive rate of TPOAb in patients with chronic urticaria was higher than that in the control group,with a statistically significant difference [OR=6.93(95%CI :3.57-13.42,P < 0.01)].Subgroup analysis was conducted according to different TPOAb detection methods.Except electrochemical luminescence [OR=5.20(95%CI :0.66-41.16,P=0.12)],other detection methods showed that the positive rate of TPOAb in patients with chronic urticaria was higher than that in the control group,with statistically significant difference [radioimmunoassay: OR=7.16(95%CI :3.87-13.24,P < 0.01);elisa:OR=10.82(95%CI: 5.17-22.65,P < 0.01);chemiluminescence: OR=4.85(95% CI:1.45-16.24,P < 0.01)].According to the regional distribution of the population,the subgroup analysis showed that the positive rate of TPOAb in the Asian group and the European group was higher than that in the control group,and the difference was statistically significant [Asia: OR=5.62(95%CI: 2.67-11.84,P < 0.01);Europe: OR=10.82(95%CI :5.81-20.16,P < 0.01)].(3)There were 24 case-control studies satisfying TGAb and chronic urticaria,the results of meta-analysis showed that the TGAb positive rate of chronic urticaria patients was higher than that of the control group,and the difference was statistically significant [OR=7.78(95%CI: 4.79-12.99,P < 0.01)].According to different TGAb detection methods,the TGAb positive rate of the patients with chronic urticaria was higher than that of the control group,and the difference was statistically significant[electrochemicalluminescence: OR=10.36(95%CI: 2.65-40.45,P < 0.01);radioimmun oassay: OR=5.05(95%CI: 2.69-9.47,P < 0.01);elisa: OR=15.56(95%CI :5.60-43.22,P < 0.01);chemiluminescence: OR=9.54(95%CI: 3.88-23.49,P < 0.01);agglutination test: OR=26.95(95%CI :1.49-488.33,P= 0.03)].According to the regional distribution of the population,the subgroup analysis showed that except the American group [OR=9.99(95%CI :0.52-191.90,P=0.13)],the Asian group and the European group had a higher TGAb positive rate than the control group,with statistically significant differences [Asia: OR=7.45(95%CI :4.19-19.27,P < 0.01);Europe: OR=9.22(95%CI :3.19-19.27,P < 0.01)].(4)The sensitivity analysis showed that the results of this meta-analysis were relatively robust and reliable.(5)Funnel plot and Egger's regression both indicated publication bias.Conclusion :(1)TPOAb was correlated with the incidence of chronic urticaria;(2)TGAb was correlated with the incidence of chronic urticaria;(3)Due to the limitations of current research types,quantity and quality of literature,the above conclusions still need to be confirmed by furthermulticenter,large-sample and high-quality studies.
