Objective:To study epidemiological and spatial distribution characteristics of human brucellosis of Jilin Province so as to provide scientific basis for prevention and control strategies settlement and adjustment,the thesis analyzed the three distribution,epidemic process and spatial autocorrelation of the new human brucellosis cases in Jilin Province from 2014 to 2019.Methods:Using Excel 2019 software,the epidemic trend of human brucellosis cases in Jilin Province from 2014 to 2019 reported in National information management system for infectious disease reporting was described.Excel 2019 software were used to describe the characteristics of Three Distribution of the case information of 8,385 new human cases reported by the first institute for endemic diseases control and prevention of Jilin Province from 2014 to 2019.Arcgis 10.2 and Geoda 1.14.0software were used to conduct trend analysis and spatial autocorrelation analysis of human brucellosis in 64 counties(cities and districts)in 9 cities(prefectures)of Jilin Province from 2014 to 2019.Results:A.Epidemiological characteristics of human brucellosis in Jilin Province from2014 to 2019(a)A total of 8,385 human brucellosis cases were reported in Jilin Province from2014 to 2019,with an average annual incidence of 5.31/100,000,showing an overall downward trend.The highest incidence was in 2014(6.58/100,000),slightly higher in2018(4.44/100,000),and lowest in 2019(4.09/100,000).(b)Human brucellosis cases were reported in all cities(prefectures)of Jilin Province from 2014 to 2019.The highest incidence was in Baicheng city(23.29/100,000),followed by Songyuan city(15.54/100,000),accounting for 64.83%.Jilin city had the lowest incidence(1.18/100,000),followed by Baishan city(1.22/100,000).(c)Human brucellosis cases occurred in all months from 2014 to 2019 in Jilin Province,mainly from march to July.The highest incidence month was may,with1,060 cases(12.64%)reported.The lowest was in December,with 351 cases(4.19%)reported.(d)A total of 6,219 male and 2,166 female cases of human brucellosis were reported in Jilin Province from 2014 to 2019,with a sex ratio of 2.87:1.Cases have been reported in all age groups.The number of patients aged 40 to 59 accounted for59.45%.In terms of age distribution,the incidence of brucellosis occurred in all stages,among which the highest age group was 45-year-old old group,with 1,523 reported cases,accounting for 18.16%.The second group was the 50-year-old group,with 11,457 reported cases,accounting for 17.40%.The number of cases in the40-year-old group and the 55-year-old ~ group was 1,022 and 981,respectively,accounting for 12.19% and 11.70%.The number of patients aged 1 to 19 years and over 75 years old was less,accounting for 3.20% of the total cases.The human cases were reported in all occupations.The largest number of cases were reported was farmers(6,889 cases,accounting for 82.16%),followed by herdsmen(684 cases,accounting for 8.16%),and domestic and unemployed workers(304 cases,accounting for 3.63%).The cases of three occupational diseases account for 93.95% of the total cases.(e)The main source of infection of human brucellosis in Jilin Province from2014 to 2019 was sheep,with 7,693 infected cases,accounting for 91.75% of the total cases.The transmission factors were mainly flow products and droplets,and the infection cases accounted for 50.65%(4247/8385)and 34.73%(2912/8385)of the total number of cases,respectively.In addition,480 cases due to fur infection accounted for 5.72%,412 cases due to food infection accounted for 4.91%,and 334 cases due to other factors accounted for 3.98%.The transmission route was mainly through the skin,mucous membrane and digestive tract.The infected cases were4,975 and 2,932,accounting for 59.33% and 34.97% of the total cases.In the history of exposure,the top four were clearing the enclosure,grazing,lambing,and living in the epidemic area.The number of cases was 5,736,5,388,4,972 and 3,570,accounting for 68.41%,64.26%,59.30% and 42.58% of the total cases,respectively.The protective measures adopted include special working clothes,masks,rubbergloves,boots and protective equipment soaked with disinfectant,with the composition of 14.17%,27.82%,27.87%,14.00% and 14.56% respectively.B.Spatial distribution characteristics of human brucellosis in Jilin Province from2014 to 2019(a)The analysis on the trend of human brucellosis in Jilin Province from 2014 to2019 shows that the trend of brucellosis in each year is basically the same,which increases from east to west and from south to north.(b)Global autocorrelation results show that the global Moran's ? index ranged from 0.529 to 0.802,and P< 0.05,the Moran's I was not 0.The spatial autocorrelation of human brucellosis incidence in 9 city(state)64 counties(city,area)is significant and positive,adjacent cities shared high and low incidence,there exists a significant spatial agglomeration phenomenon.(c)The results of local spatial autocorrelation analysis of human brucellosis from2014 to 2019 in Jilin Province showed that the high-to-high aggregation areas in 2014,2015,2016,2017,2018 and 2019 were all distributed in the northwest,among which,Taoan city,Zhenlai county,Daan city,Taobei district,Tongyu county,Changling county,Qian 'an county and Qianguo county were the high-incidence areas.The incidence of low to low concentration areas have changes every year,mainly for Jiutai city,Kuancheng district,Lvyuan district,Erdao district,economic and technological development district,Shuangyang district,Longtan district,Changyi district,Chuanying district,Fengman district,Changbai Korean autonomous county.Conclusion:A.The incidence of human brucellosis in Jilin Province generally shows a downward trend from 2014 to 2019,with the highest in 2014,the lowest in 2019,and a small increase in 2018.B.The incidence of human brucellosis in the northwest of Jilin Province from2014 to 2019 was significantly higher than that in other regions,presenting obvious seasonality,with high incidence from march to July.C.Human brucellosis cases in Jilin Province from 2014 to 2019 were mainly male,aged 40 to 59 years old,and mainly occupation as farmers and herdsmen.D.The main source of infection of human brucellosis in Jilin Province from2014 to 2019 is sheep,and the transmission factors are mainly flow products and droplet dust,and the transmission routes are mainly skin mucosa and digestive tract.Sweep enclosure,grazing,lambing as the main exposure history.E.The incidence of human brucellosis in Jilin Province from 2014 to 2019 showed a positive spatial correlation with significant spatial clustering.F.The counties and districts under the jurisdiction of Baicheng city,Changling county,Qianguo county and Qian 'an county of Songyuan city are the regions that need to prevent and control the high incidence of brucellosis among human beings,and sufficient attention should be paid.
