Objective:Through the application of Longzhong Xiaozhong Zhitong Mixture to Osteofascial Compartment Syndrome(OCS)in patients with grade ? and ?,to observe the influence on intracompartmental pressure(ICP),tissue oxygen and serum creatine kinase(CK),lactate dehydrogenase(LDH),and glutamate aminotransferase(AST)levels of limb muscle and to prove Longzhong Xiaozhong Zhitong Mixture could improve the supply of oxygen to wound tissue,decrease oxygen,eliminate the swelling and alleviate pain,effectively control the occurrence of OCS related complications,and provide safer and more effective treatment for the majority of patients with OCSMethods:In this study,a total of 106 patients were randomly divided into two gr oups by computer digital production process,who were diagnosed as osteofascial compa rtment syndrome(grade ? and ?)in Gansu provincial hospital of traditional Chinese medicine due to crush injury of legs,closed tibial plateau fracture,closed tibial plateau fracture,and closed tibiofibular fracture from October 2019 to January 2018,53 cases in treatment group(including 12 cases of closed tibial plateau fractures,19 cases of clos ed tibial fracture,22 cases of closed tibiofibula fractures),53 cases in the control group(including 14 cases of closed tibial plateau fractures,18 cases of closed tibial fracture s,21 cases of closed tibiofibula fractures).After the patients were enrolled,the control group was given the routine treatment of osteofascial compartment syndrome(includin g diuresis detumescence,rehydration,prevention of reperfusion injury,antibiotics to preven t of infection,and so on).On the basis,the treatment group were received Longzhong X iaozhong Zhitong Mixture prepared by Gansu Provincial Hospital of TCM.Then,near i nfrared spectroscopy(NIRS),measurement of ICP,peripheral diameter measurement of fix ed position of limbs were performed respectively in the two groups before treatment a nd 3,7 and 14 days after treatment,and serum creatine kinase(CK),aspartate aminotransf erase(AST)and lactate dehydrogenase(LDH)were detectedResults:1.There was no significant difference in baseline data(gender,age,fracture site and type)between the two groups(P>0.05),suggesting comparability.2.In the tw o groups,perfusion pressure difference AP reached the lowest in three days after treatm ent,AP were recovered in 2 weeks after the treatment,and the treatment group recovere d better ?P7d<0.05,?P14d<0.05.3.The tissue oxygen saturation first declined after tra uma,became the lowest in 3 day in the two groups,there was no statistically significan t difference,both recovered in some extent in 7d and 14d after treatment,and the treat ment group increased more obviously ?P14d<0.05.4.Compared with pre-treatment,the biochemical indicators of CK,LDH and AST in the two groups all showed a decreasi ng trend,and the decrease was faster in the treatment group than in the control group(P<0.05).5.After treatment,12 patients in the treatment group and 20 patients in the control group progressed to grade III,with statistically significant differences(P<0.05).6.The effective rate was 82.1%in the treatment group and 67.9%in the control gro up according to the symptom scale,and the difference was statistically significant(P<0.05).Conclusion:Longzhong Xiaozhong Zhitong Mixture for OCS could effectively relieve intracompartmental pressure,improve the oxygen supply of tissue,and reduce the serum levels of CK,LDH and AST.
