The rural physically disabled rehabilitation is an important part of rehabilitation work of the disabled,at the same time,it has also became a focus in the work of the government.However,from the current situation of rehabilitation of the physically disabled in the rural areas,there is still great rehabilitation demand and the existing services can not meet this demand.In addition,because of the particularity of the rural areas such as the lower level of economic development,the unsound rehabilitation security system,lower rate of the limb disabled labor participation,the inconvenient transportation and resource access and the more closed and conservative idea of rural people,all of these that make the rural rehabilitation level lags behind,which further reflects the reality of the hardship of disabled rehabilitation in rural areas.In this paper the writer chooses the disabled people in rural areas in Wuwei city as the research object,with one of them as the representative example and by using the action research,through the study for nearly five years of practice,to show the whole procedure of how to use the case management –one of the service modes for the disabled of their social rehabilitation.The research is carried out around the following link: the selection of the client,the client-needs analysis,planning,resource links,and specific implementation process and,the service result evaluation and in the end of the paper the writer concluded that case management is effective in rural rehabilitation services for the disabled and there are also challenges and difficulties,for which the writer mentioned some introspection of the research and the Suggestions for the readers.
