Faced with the fact that China's rural aging is intensifying and medical burdens are increasing and families with the old are difficult to support themselves and the country is increasingly paying attention to national health,combining the author's discovery of strong demand for health and longevity from rural seniors during the author's period of life and learning,after analyzing the domestic and international theory of gerontology,the article selects health aging as the perspective of research and pays attention to the health of the old in rural communities.It focuses on practical research and practical intervention,and selects D village in the northeastern region as the research object.The article analyze of the current situation of the health management of the old,and find that the health management of the old in the village is facing the double threat of internal and external,including: individuals with poor health awareness,bad habits,lacking of nutrient and health knowledge,threats from their living environment.On this basis,the article selectes eight non-healthy elderly people in the village as the intervention targets,and establishes an elderly health management group to provide health education for the elderly,intervention of health risk factors,and construction of a healthy family support network.After the work of the group was completed,social worker intervened on a member of the group who had quit smoking.By assessing the use of behavioral therapy,the smoking behavior of the case-client was improved.The method using the SF-36 scale is measuring the health status of the clients before and after the intervention.The data of the health indicators of the service objects after participating in the practice has increased significantly,confirming the effectiveness and feasibility of social work intervention in the health management of the elderly in rural communities.The effectiveness of social work interventions for the health management of elderly people in rural communities also confirms that “prevention-oriented” health management and “helping people to help themselves” have a cooperative advantage.Through the effective combination of the two professional services,the health of the elderly has been improved.This can not onlyreduce the economic expenditures of families,units,and society on medical and health care,but also make the physically and mentally healthy elderly people fully utilize the residual heat and join in the social reproduction.Although the article has shown initial success in the health management of the elderly in the community,how to promote the operation of community elderly health management still requires the joint efforts of the government and society.The smooth promotion of health management in rural areas still needs to rely on the maturation and perfection of related emerging professions such as health management and social work.In addition,social advocacy on health knowledge,health investment should also be important ways to reduce the misunderstanding of health management in rural and poverty-stricken areas.
