Study On The Application Of Case Management In Home-based Care For The Elderly People With Disabilities In Urban Community

With the continuous development of the process of aging society in China,the situation of pension is increasingly grim,how to improve the quality of life of the elderly,so that the elderly better to spend their old age,become a national and social important issue.Home-based care is not only the supplement and development of traditional family pension mode in China,but also an active attempt and exploration of China's social coping with old-age pressure in the new era.However,in our country's existing home-based care service system,the disabled elderly are in a weak position due to the dual attributes of old age and disability,and their diversified pension demands are difficult to meet,which leads to the difficulties in realizing the old-age security of the disabled elderly group.Among the disabled elderly groups,the social support network is more obvious than the disabled in the rural areas,and the home-based care is more expensive,and the implementation environment is more complicated than the disabled elderly in the urban community.Case management is a social work method that provides integrated services for individuals or families who are confronted with multiple needs and problems and cannot make proper use of resources to improve their situation,which can effectively reduce the cost of social services and meet the multiple demands of the clients.This article takes the case management as the method,uses the case management pattern to intervene the two disabled old person's home-based care service as the example,detailed exploration case management method is how to coordinate,plan as a whole,and links to resources to help the disabled old person to solve the home-based care aspect actual predicament,so as to analysis case management in response to and meet the demand of urban community ofdisabled elderly home-based care service aspects of the advantage and application effect,reflect the difficulties and obstacles encountered during operation,and on the basis of exploring the solutions and strategies.It is found that case management is helpful to meet the diversified needs of the elderly people with disabilities in urban community,compared with the general case work,the case management model can more effectively integrate the system resources to help the service object to solve the real plight of the home-based care,reduce the pressure of family care,strengthen the social support network system,improve the quality of disabled old people in later life.In addition,the lack of current social service welfare resources and the imperfect service system are the important factors that affect the successful implementation of case management,and there is still a long way to go before the case management model is widely used.
