Objective:This paper investigates the willingness and reasons of the elderly to participate in long-term care insurance in Qinhuangdao,analyzes the factors affecting the willingness of the elderly to participate in long-term care insurance,and provides reference for the formulation of long-term care insurance policies in Qinhuangdao.Method:April to May 2019,adopting the stratified cluster random sampling method,the harbour area,Shanhaiguan,Beidaihe,Funing area,economic and technological development zone,Beidaihe district,Changli county,Lulong,Qinglong Manchu autonomous county,9 sample areas,extracting eligible 450 elderly people,through to the elderly in long-term care insurance intention questionnaire survey.T test was used for the analysis of questionnaire items.Cronbach's ? was used for reliability evaluation,and Spearman correlation coefficient was used for retest reliability.Surface validity and expert content mean validity S-CVI index were used for validity evaluation.SPSS21.0 software was used for descriptive statistical analysis and statistical inference of survey data.Counting data were expressed by frequency and percentage(%).Univariate analysis was performed by chi-square.Variables with statistically significant differences in univariate analysis were included in Logistic regression analysis to screen the influencing factors on the participation of the elderly in long-term care insurance.P<0.05 was considered statistically significant.The open entry and additional entry of the elderly who are willing to participate in long-term care insurance were coded,summarized,and then descriptive statistical analysis was used.Results:(1)Questionnaire item analysis t=-38.807,the difference was statistically significant(P<0.05).The reliability coefficient Cronbach's?=0.741,and the reliability coefficient Spearman was 0.861.A preliminary survey was conducted on 30 elderly people.The average time to fill in each questionnaire was about 9 minutes,and the I-CVI index of expert content validity ranged from 0.8 to 1,S-CVI =0.949.(2)There were 270 elderly(60.5%)who were willing to participate in long-term care insurance.The insurance compensation was mainly cash or nursing services(156 people,57.8%),among which the cash payment amount was mainly > 2000 yuan/month(59 people,26.1%).There are 124people(45.9%)who are willing to pay,among which the proportion of voluntary contributions is mainly 11%-20%(19 people,15.3%),and the amount of voluntary contributions is mainly 101-500 yuan/year(29 people,23.4%).108(40.0%)listed reasons for their willingness to participate,including reducing family burden(46 people,42.6%),ensuring elderly living(32 people,29.6%),providing care services(15 people,13.9%),sharing care costs during medical procedures(9 people,8.3%),and responding to national policies(6 people,5.6%).72 people(41.1%)listed reasons for their unwillingness to participate in insurance,including good health,future family care(24 people,33.3%),existing medical and business insurance(25 people,34.7%),financial difficulties(18 people,25.0%),old age,lack of insurance(5 people,6.9%),and not understanding long-term care insurance policies(3 people,4.2%).(3)Single factor analysis showed that different demographic characteristics(age,educational background),family characteristics(the presence of a spouse,living conditions,children count,son,main caregivers),health(self-conscious health condition,the number of chronic disease,nearly a year in the hospital times),economy(the main economic source,monthly income,health care),Ideology(care place preference)of the elderly in long-term care insurance will difference was statistically significant(P< 0.05);Logistic regression analysis showed that age,spouse,number of sons,primary caregivers,number of chronic diseases,number of hospital stays in recent one year,and monthly income were the influencing factors for the elderly to participate in the long-term care insurance(P<0.05).(4)The interview results show that the president of the nursing home,the head nurse of the hospital and the manager of the day care center all believe that the long-term care insurance can effectively solve the care problems of the elderly,and the establishment of the long-term careinsurance policy should be accelerated.Conclusion:(1)Elderly people in Qinhuangdao are not willing to participate in long-term care insurance.(2)The influencing factors of long-term care insurance among the elderly in Qinhuangdao were age,spouse,number of sons,number of primary caregivers,number of chronic diseases,number of hospitalization in the past year,and monthly income.(3)Older people with low age,no spouse,few sons,no one to take care of them,a high number of chronic diseases,a high number of hospitalizations in the past year,and a high monthly income are more willing to participate in long-term care insurance.
