Population aging has become a worldwide problem facing mankind.Population age structure presents an inverted pyramid trend.Under the influence of family planning policy,the family model in China is becoming smaller and smaller.Traditional multi-generation co-habitation has evolved into the present "421 family model".Under the dual impact of family miniaturization and population aging,the present situation of the elderly's pension is even worse than in the past.In recent years,China's economic development and urban construction have been accelerating.Urbanization is the potential and momentum of China's domestic demand.At present,58.5% of China's urbanization rate still lags far behind the average level of 80% in developed countries.Therefore,the product of "Village transfer" community,a dual system of urban and rural areas,will exist for a long time.In the process of urbanization,the living "Village transfer" community residents have been greatly changed.Especially for the elderly groups,with the change of living environment,there are various problems such as lack of pension and medical security,community integration,spiritual consolation and so on.This research is put forward against the background of accelerating the aging process,increasing "Village transfer" communities and gradually being accepted by the public in the process of urban development.This study chooses D community,a typical "Village transfer" community in Xi'an,as the research object.Through a questionnaire survey and unstructured interviews on the needs and status quo of home-based care for the elderly in D community,the author finds that: At present,there is a big deviation between the home-based pension service provided by the community and the home-based pension demand of the elderly in the community,which is mainly reflected in that the economic base and living structure of the elderly have changed,and the consequent demands for medical security and spiritual comfort have not been met,There is a great demand for community participation and integration of the elderly,and the community does not provide this type of home-based pension services.As a professional social worker,the author has worked in D community for two years,and can accurately assess the needs and service status of the elderly in D community.On this basis,the author carries out group work and community work intervention to provide home-based care services for the elderly in D community.The contents include: To carry out group work to meet the needs of spiritual consolation and community participation for the elderly,and to carry out community activities to improve the quality of community health care and spiritual and cultural services.Social workers Intervention in D "Village transfer" community home-based pension service has achieved some results,and targeted to meet the needs of the elderly home-based pension,but there are still many shortcomings.The author draws the following reflective conclusions: despite the needs survey,there is no need file for the elderly,insufficient service pertinence and personalized design,community volunteer team has been established,but professional social workers are insufficiently staffed,and the long-term service mechanism between community home care and community organizations has not been established.Based on the above reflection,the author was corresponded suggestions for three aspects: service object,community workers and the construction of home-based pension service mechanism,hoping to enrich and improve the construction of home-based pension service for the elderly in the "Village transfer" community of Xi'an,and to form a certain reference significance for other areas to improve the home-based pension service mode.
