Objective:The purpose of this study was to investigate the effects of 4-week 3-D and 2-D dynamic balance training on postural control in individuals with functional ankle instability(FAI),including:1.Comparing the effects of three-week 3-D dynamic balance training with traditional 2-D balance training for the posture control of functional ankle instability.2.Analyze how dynamic balance training affects the balance strategy of maintaining posture stability,three sensory strategies and motion control ability;Method:According to the inclusion criteria of functional ankle instability proposed by the International Ankle Joint Association,20 patients with functional ankle instability were selected from the Rehabilitation Department of Huadong Hospital.The patients were randomly divided into two groups,and each group contained 10 cases.The experimental group is trained by a three-dimensional dynamic balance training device(3-D group).The anther group used the traditional stable support surface balance training instrument(2-D group).The two groups were trained for four weeks and trained four times a week for about 30 minutes each time.The subjects' dynamic balance ability,motor control ability,single-leg stance ability,and ankle stability self-evaluation scale were evaluated before and after training.SPSS 23 statistical software was used for data processing.Paired sample t-test analysis method was used to compare intra-group differences.Independent sample t-test analysis method was used to compare differences between groups.The significance level was defined as p<0.05 as significant difference.Result:1.Dynamic balance ability test results:After 4 weeks of training,in terms of sensory organization ability,the composite scores of both 3-D group and 2-D group were improved compared with those before training(p<0.05),but the change of composite score of the 3-D group were higher than those in the 2-D group(p<0.05).The first,second,fourth,and sixth equilibrium scores of the 3-D group were improved after training(p<0.05).Balance strategy:After 4 weeks of training,the strategy scores of the 3-D group in conditions 4,5 and 6 were higher than those before training(p<0.05),and those in the 2-D group were increased in condition 5(p<0.05);The change score of 3-D group was higher under the conditions 6 than the 2-D group(p<0.05).Sensory weight:The ability of the 3-D group to ignore false visual information was significantly higher than that before training(p<0.05).2.Motion control ability:When the balance plate was twitched backward,the latency time of the 3-D group was shorter than that before training(p<0.05).When the balance plate was twitched forward,the amplitude scaling of both groups was smaller than that before training(p<0.05).In the inter-group comparison,when the balance plate was slide backward,the latency time of the 3-D group was shorter than that of the 2-D group(p<0.05).3.Single-leg stance:In the 3-D group,the sway velocity of the affected foot decreased when with or without vision compared with that before training(p<0.05).When eyes open,the sway velocity of the 2-D group was less than that before the training.(p<0.05).4.Cumberland Ankle Instability Tool:After 4 weeks of training,the Cumberland Ankle Instability Tool scores of both two groups were higher than those before training(p<0.05),but the effects of 3-D group were not better than the 2-D group(p>0.05).Conclusion:1.Four-week dynamic balance training can improve the composite balance ability of FAI patients,the overall sway amplitude is reduced,and the single-leg support ability is improved,and the posture control ability is improved.The 3-D dynamic balance training effect is better than the traditional 2-D balance training.2.Four-week 3-D dynamic balance training can improve the motor control ability of FAI patients,reducing visual dependence,and enhance the utilization of vestibular sensation.Which helps FAI patients to coordinate and select sensory information more effectively to maintain posture stability.
