The competition in today's society is extremely fierce, and the proportion of patients with various mental disorders is increasing year by year. These patients need professional people to help guide them to recover and return to society in the middle or later stages of treatment. There are groups affected by mental disorders in the community.Patients with mental disorders need to take medicine for a long time to control their illness.Generally, adverse reactions will occur after taking medicine.Patients with better recovery can work normally. However, most patients with mental disorders have problems in social adaptation, including inferiority complex, unwillingness to contact with people and separation from society.Over time, they cannot integrate into the society again, which is very disadvantageous to their recovery. At this time, someone needs to lead this group out of their homes and reintegrate into society. At present, the general treatment plan for patients with mental disorders is hospitalization and medication, but this traditional treatment plan treats patients on the one hand, and on the other hand, the closed treatment environment and single biological drug treatment isolate patients with mental disorders from the outside world, limiting their right to communicate with others. In the long run, patients with mental disorders are prone to repeated illness, and cannot get a fundamental recovery. They are still unable to integrate with the society. The intervention of social workers in the later rehabilitation of patients with mental disorders is of great help to their psychological changes and improvement of living standards. This article takes Xiao Meng,a mentally handicapped patient in Shenzheng,as an example, and discusses the process of social workers intervening in the late rehabilitation of mentally handicapped patients through case management.Social workers use unstructured interviews and observation methods to understand the basic situation, problem needs and rehabilitation difficulties of the service objects. They make rehabilitation plans for the service objects, improve the life and workskills of the service objects, increase the social and professional skills of the service objects, enhance self-confidence,improve the family support system and return to society.
