Case Management Study Of Hospice Care For Cancer Patients

As the incidence of cancer continues to rise,the hospice care of patients with cancer is gradually receiving widespread attention in society.At the same time,patients with cancer at the same time suffer enormous physical,psychological and economic pressures,and face multiple and complicated difficulties.The current situation of hospital work mode is difficult to meet the needs of patients with cancer at the end of the period.Medical social work can make up for hospital services.Insufficient.Through the collation of related literatures,the research on social work involved in hospice care services is mostly carried out in the form of cases and groups.There are few studies on the methods of case management involved in hospice care services.The case management model can integrate social resources,solve complex problems,and provide more comprehensive and effective services for patients with cancer.It is very suitable for hospice care for patients with cancer.The research object of this paper is the patient with cancer.The author relies on the practical experience of intervention in the oncology department of a hospital in Shanxi,and provides hospice care service based on Maslow's hierarchy of needs theory and social support theory.This paper selects two cases and introduces and reflects on the work process by showing the social work method of case management to intervene in the actual process of cancer patients.In the early stage of the service,the author obtained the trust of the client and his family through the establishment of professional relationships,conducted problem analysis and needs assessment,and analyzed the existing resources available to the client.In the service process,assist the client to remove barriers to the use of resources,link,integrate social resources,and cultivate the ability of the client to use resources.Use social resources to help the client adjust negative emotions,relieve physical and mental stress,provide financial support and spiritual care,solve the difficulties faced by the client,and provide hospice care services involving the whole body and mind.Through the practical exploration of the hospice care service for patients with cancer,it is not difficult to find that the case management model is involved in the hospice care service and its feasibility and necessity.After sorting out and reviewing the practice,the author found that using the case management model to provide caring patients with end-of-life care for the end-of-life care can give full play to the greatest advantage of team service,provide better quality hospice care,and improve the quality and significance of the dying life of the client..At the same time,we cannot ignore the improvement of the quality and ability of social workers as case managers.Finally,we propose development proposals,including strengthening the construction of medical social workers' talent team,promoting the joint development of case management mode and social work.Society creates a good environment full of human care.
