Part I:Development of a regulable model of benign bile duct stricture in miniature pigs.[Objective]To investigate the animal modeling of biliary stricture which is accurately controllable.[Methods]The data of 24 cases with biliary stricture was retrospectively analyzed from 2015 to 2016.24 pigs were divided into four groups by surgical treatment(group A as minor-caliber group,group B as heavy caliber group,group C as conventional group,and group D as control(Sham surgery)group,every group with 6 cases).Group A and B were treated with the controllable method to accurately regulate the degree of bile duct stenosis by placing the stent in the bile duct and suture.Group A used the stent with 3.0 mm outer diameter.Group B used the stent with 6.0 mm outer diameter.Group C,the conventional group was treated by suturing lower bile duct with 5-0 absorbable suture and measure the degree of bile duct stenosis with eyeballing.Group D was treated by sham surgery,only expose the bile duct with no suture.The common condition and liver function level of the animals were observed.Hepatic function tests were conducted on femoral blood of four groups taking in preoperative and postoperative 1,3,7,14,21,28 day.Cholangiography via bile duct was conducted in postoperative 30 day.The four groups were executed and dissected in postoperative 30 day to observe the histological change of liver and bile duct.[Results]Minor-caliber group's pigs all survived the surgery in postoperative 30 day.Jaundice appeared in postoperative 5-7 day and aggravated after.Obvious rise of TB was detected in postoperative 28 day.Cholangiography via bile duct on 6 pigs in postoperative 30 day showed obvious visible stenosis of different degree with 2.7 mm average diameter(from 2 mm to 4 mm).Chronic fibrosis scars were observed in pathological anatomy.The heavy caliber group's pigs all survived the surgery in postoperative 30 day.Yellow staining on the skin of abdomen appeared in postoperative 14 day.Cholangiography via bile duct on 5 pigs in postoperative 30 day showed obvious visible stenosis with 5.5 mm average diameter(from 5 mm to 7 mm).Mild lesions of bile duct mucosa were observed in pathological anatomy.[Conclusion]This is an comparatively ideal animal model for benignant biliary stricture with the advantages of high repeatability and tolerability.The model can be used to provide reliable experimental basis for researches on biliary stricture.Part ?:Application of a new type degradable magnesium alloy bile duct stent in porcine benign bile duct stricture.[Objective]A new type degradable magnesium alloy stent was placed in the injured bile duct of pigs to evaluate the clinical effectivity and safety of the stent on the treatment of benignant biliary stricture.[Methods]The animals were divided into two groups by degradation speed of the degradable bile duct stent(group A as fast degradation group and group B as intermediate degradation group).12 pigs were treated the way minor-caliber group was treated in part I.Numbered the animals and divided them into two groups at random,6 for each group.Stents of different quality were randomly allocated among 12 pigs.[Results]The animals of two groups all resuscitated from the surgery and started eating in 12 hours after resuscitation.The data of 12 cases came into analysis.In two cases of the fast degradation group,decrease of diet appeared in postoperative 2 day,stopped eating in postoperative 5 day,observed of severe yellow stain on skin and sclera and obvious rise of TB.The two cases suspected of having bile duct obstruction were executed in postoperative 1 week.In one cases of the intermediate degradation group,bile duct obstruction happened in postoperative 7 day.The rest of this group recuperated well from the surgery.The stents degraded completely.There were no complications like bile leakage and obstruction.[Conclusion]JDBM degradable magnesium alloy bile duct stent is effective in the treatment of benignant biliary stricture.The intermediate degradation stent have the therapeutical effects in the treatment of benignant biliary stricture to some extent.But the stent still needs further improvement.
