ObjectivesThrough three-step acupuncture therapy for dysphagia caused by post-stroke pseudobulbar paralysis,and parallel control study with swallowing rehabilitation training,the clinical efficacy of three-step acupuncture therapy for dysphagia caused by post-stroke pseudobulbar paralysis is determined,and a safe and effective comprehensive treatment scheme is provided for clinical treatment.MethodThis study adopts prospective randomized parallel grouping control design.According to the course of dysphagia less than 1 month and 1 month and above,it is divided into 2 layers.SAS statistical software is applied to compile a random allocation scheme for two groups of stratified area groups.104 patients with dysphagia due to pseudobulbar paralysis after stroke who meet the inclusion criteria are divided into an experimental group and a control group,52 cases in each group,2 cases in the treatment group and 5 cases in the control group.All patients were given basic treatment for cerebrovascular diseases.The experimental group received scalp acupuncture and neck acupuncture plus swallowing rehabilitation training,while the control group received swallowing rehabilitation training five times a week for a total of four weeks and followed up for three months.Clinical evaluation was carried out at 2 weeks and 4 weeks after intervention respectively,and follow-up was carried out 3 months after treatment.The improvement of swallowing function,quality of life and ability of daily living of patients with dysphagia after stroke were evaluated by using standard swallowing function scale,dysphagia specific quality of life scale and Babbitt score scale.Result1.There was no significant difference between the two groups before treatment in terms of sex,age,course of disease,stroke type and other basic data(p<0.05),which was comparable.2.Comparison between the two groups in improving swallowing function:at the end of 4 weeks of treatment,SSA scores of the two groups were compared,and the difference was statistically significant(P<0.05).It shows that the improvement of dysphagia symptoms by three-step acupuncture is better than simple rehabilitation training.After 2 weeks of intervention,the difference in scores between the two groups was statistically significant(p<0.05),indicating that the three-step acupuncture method can shorten the improvement time of swallowing function.At 3 months follow-up,the difference in scores between the two groups was statistically significant(p<0.05),indicating that the long-term effect of three-step acupuncture method in improving swallowing function is better than that of simple rehabilitation training.3.Comparison between the two groups for improving swallowing quality of life:at the end of treatment,the SWAL-QOL scores of the two groups were compared,and the difference was statistically significant(P<0.05).It shows that the improvement of swallowing quality of life by three-step acupuncture is better than simple rehabilitation training.After 3 months,the difference in scores between the two groups was statistically significant(p<0.05),indicating that the three-step acupuncture method is superior to simple rehabilitation training in improving the long-term effect of patients' swallowing quality of life.4.Comparison between the two groups in improving daily living ability:Barthel index scores of the two groups were compared at the end of treatment,and the difference was not statistically significant(P>0.05),indicating that three-step acupuncture and swallowing rehabilitation training had no obvious difference in improving patients' daily living ability.5.Comparison of the effective rates between the two groups:the total effective rates of the three-step acupuncture group and swallowing rehabilitation training group were 98.00%and 74.47%respectively,with significa nt difference(P<0.05)and statistical significance.This shows that the clinical effect of three-step acupuncture on patients with dysphagia due to pseudobulbar paralysis after stroke is better than simple swallowing rehabilitation training.6.Comparison of clinical effects between the two groups of patients in acute phase:at the end of treatment,the difference between the two groups in SSA score and SWAL-QOL score was statistically significant(P<0.05),which showed that the improvement of swallowing function and quality of life of patients with dysphagia in acute phase by three-step acupuncture was better than that by simple rehabilitation training.7.Comparison of clinical effects between the two groups of convalescent patients:at the end of the treatment,the differences between the two groups in SSA score and SWAL-QOL score were statistically significant(P<0.05),indicating that the improvement of swallowing function and quality of life of convalescent dysphagia patients by three-step acupuncture is better than that by simple rehabilitation training.Conclusion1.Three-step acupuncture can effectively improve the swallowing function of patients with pseudobulbar palsy dysphagia after stroke,shorten the time for improvement of swallowing function,and improve the quality of life of patients.Its long-term curative effect is positive,and the clinical curative effect of three-step acupuncture is better than simple swallowing rehabilitation training.2.The three-step acupuncture method has a good therapeutic effect on dysphagia due to pseudobulbar paralysis after stroke in both acute and convalescent stages.
