Objective: To observe the clinical efficacy of different integrated Chinese and western medicine schemes in the treatment of vascular dementia with kidney deficiency and blood stasis syndrome,to evaluate its role in improving symptoms,cognitive function and the ability of daily living activities,and to explore relatively optimized treatment schemes.Methods: Based on the principle of randomization and control,100 patients meeting the inclusion criteria were randomly assigned to four groups,namely Group A(western medicine treatment + chinese medicine decoction),Group B(western medicine treatment + chinese medicine decoction + moxibustion),group C(western medicine treatment + chinese medicine decoction + acupuncture),and group D(western medicine treatment + Chinese medicine decoction + Chinese medicine injection),which had 25 patients in each group,according to the proportion of 1:1 with random number table method.The four groups all took 8 weeks as a course of treatment.All patients were scored with Mini Mental State Examination(MMSE),Activity of Daily Living(ADL),Chinese medicine syndrome before and after treatment,and recorded safety indicators such as four tests,blood routine,coagulation function,liver function,renal function,myocardial enzyme,electrocardiogram and other adverse reactions or events during the trial.Results: 1.All four groups could improve the scores of the MMSE with significant differences(P<0.01).The total scores of groups B,C,and D were higher than those of group A,and the improvement was more obvious in the group C(western medicine treatment + chinese medicine decoction + acupuncture),and the difference was statistically significant compared with group B and group D(P<0.05).MMSE was used to evaluated the clinical efficacy,the effect was the most obvious in group C with an effective rate of 81.82%.The difference was statistically significant compared with the three groups A,B,and D.2.After treatment,the ADL scores of the four groups were all lower than before,which was statistically significant compared with before treatment(PA<0.05,PB,C,D<0.01).Group C(western medicine treatment + chinese medicine decoction + acupuncture)had the lowest total score,and groups B,C,and D were lower than group A,and the difference was statistically significant(P<0.05).There was no significant difference in pairwise comparison between groups B,C,and D(P>0.05).3.All four groups can reduce the score of Traditional Chinese Medicine Syndrome Scale,the difference is significant(P<0.01).Group A has the highest total points,and group C(western medicine treatment + chinese medicine decoction + acupuncture)has the lowest,the difference was statistically significant compared with group B and D(P<0.05).The clinical efficacy was evaluated by Traditional Chinese Medicine Syndrome Scale,the efficacy of group C was the most obvious with an effective rate of 90.91%,the difference was statistically significant compared with the three groups A,B,and D(P<0.05).There was no statistically significant difference between groups B and D(P>0.05).4.There were no major adverse events during the treatment of the four groups,and they all had good safety.Conclusion: In this study,four different integrated Chinese and western medicine schemes have achieved efficacy.The combination of western medicine treatment,chinese medicine decoction and acupuncture therapy has significant effects in improving symptoms of kidney deficiency and blood stasis syndrome,cognitive function and the daily living activities. |
