Based On VEGFA/VEGFR2 To Explore The Mechanism Of Zhilian Huaban Decoction In The Treatment Of Simple Allergic Purpura

Objective and significance:To evaluate the clinical effect of Zhinv Huaban Decoction in the treatment of simple type of Henoch-Schonlein purpura with blood heat syndrome,and to detect the level of serum VEGFA before and after treatment,so as to explore the mechanism of action of Zhinv Huaban Decoction and the biological mechanism of the pathogenesis of Henoch-Schonlein purpura on the basis of the theory that VEGFA/VEGFR2 signal pathway increases vascular permeability,in order to provide a new drug target for the clinical treatment of Henoch-Schonlein purpura,and provide a scientific basis for the development of new drugs.Methods:Simple type of Henoch-Schonlein purpura with blood heat syndrome were randomly divided into two groups:the experimental group and the control group.The experimental group was treated with Zhinv Huaban Decoction orally,and the control group was treated with Rutinum Tablets and Vitamin C Tablets orally for 3 weeks,followed up for 1 month.The regression time,recurrence,TCM syndrome score and serum VEGFA level of the two groups were recorded.Finally,the differences between the two groups were statistically analyzed.The results of serum VEGFA could be compared with those of VEGF in healthy group in the literature.Results:1.Disease efficacy:The total effective rate of the experimental group was 83.3%,which was significantly better than that of the control group(52.8%)(P<0.01).Both groups can significantly reduce the total syndrome score(P<0.01),and the experimental group is significantly better than the control group(P<0.01).The recurrence rate of the experimental group was 13.9%,which was significantly lower than that of the control group(47.2%)(P<0.01).2.TCM syndrome efficacy:Both groups could significantly reduce skin purpura(P<0.01),and the experimental group was similar to the control group(P>0.05).The experimental group could shorten the course of skin purpura(P<0.01),and delay the recurrence of skin purpura(P<0.05).The scores of secondary symptoms in both groups were significantly reduced(P<0.01),and the experimental group was significantly better than the control group(P<0.01).3.Laboratory indexes:The level of serum VEGFA in both groups was significantly reduced(P<0.01),and the experimental group was significantly better than the control group(P<0.01).There was a linear regression relationship between VEGFA and main symptom score before treatment(P<0.01),and a high correlation(r=0.879),but no linear regression relationship with age,gender and course of disease(P>0.05).In acute phase,the levels of VEGF in the experimental group and the control group were significantly higher than those in the healthy group(P<0.01).In the recovery period,the level of VEGF in the experimental group was significantly lower than that in the healthy group(P<0.01),and the levels in the control group were significantly higher than those in the healthy group(P<0.01).Conclusion and significance:Zhinv Huaban decoction has a good effect in the treatment of simple type of Henoch-Schonlein purpura with blood heat syndrome.It is equivalent to Rutin Tablets and Vitamin C Tablets in the treatment of skin purpura.It has a significant advantage in inhibiting vascular inflammation,promoting vascular repair,shortening the course of disease,delaying recurrence,reducing recurrence rate and improving the overall situation.The level of serum VEGFA is highly related to skin purpura,which has double effects of damage and repair on vascular endothelium.Its recovery lags behind that of skin purpura.It can be used as an index to evaluate the vascular damage,recovery and clinical efficacy of anaphylactoid purpura.VEGFA/VEGFR2 pathway may be the biological mechanism of the pathogenesis of anaphylactoid purpura,and the mechanism of action of Zhinv Huaban Decoction in the treatment of anaphylactoid purpura,which can be further improved study its downstream mechanism.In addition to the influence of skin purpura,the level of serum VEGFA may be related to age,gender,course of disease and other factors,which may be related to the level of estrogen in different degrees in vivo.Combined with the research results of relevant literature,this paper puts forward a new hypothesis that low-dose estrogen may treat anaphylactoid purpura by regulating the appropriate expression of serum VEGFA,providing reference for further research.
