Objective:Through the general situation of 58 patients with chronic uric acid nephropathy,clinical data and the collection of syndromes,blend in laboratory test index of TCM from the Angle of molecular level detection of traditional Chinese medicine,and analyzed chronic uric acid nephropathy TCM syndrome type distribution and chronic uric acid nephropathy TCM syndrome type and the relationship between oxidative stress and renal injury index;It is hoped that this study can accelerate the excavation of the material basis of TCM syndrome differentiation and promote the standardization and objectification of TCM syndrome differentiation.Objective reference was provided for the prevention and treatment of chronic uric acid nephropathy with integrated traditional Chinese and western medicine.Methods:Selection in May 2017 to November 2019 in the third affiliated hospital of Beijing university of traditional Chinese medicine and meet eligibility criteria of the 58 patients with chronic uric acid nephropathy,kidney disease branch of China association of Chinese medicine in 2008 formulated the "uric acid of the diagnosis of renal disease,and curative effect evaluation"(trial scheme)and the two TCM physicians(subtropical high and above the title)for the final differentiation to ensure patients included in this study disease diagnosis standard,after the cases included in,according to the requirements detailed records of the patient's general condition,clinical data and samples of patient's blood and urine samples,Oxidative stress and renal injury indexes were tested,including 18 indexes,including SOD,urea(BUN),creatinine(Cr),uric acid(UA),lum 2 microglobulin(BMG),luben-1 microglobulin(AMG),retinol binding protein(RBP),lactate(LAC)electrolyte and 24h urine protein quantitative(mTP/24h).Statistical software SPSS20.0 was used to conduct data analysis on the research content of this paper,and the correlation between TCM syndrome differentiation and oxidative stress and renal injury indexes of chronic uric acid nephropathy was discussedResults:1.In this study,58 patients with chronic uric acid nephropathy were mostly aged between 60 and 69,with more males than females.2.The family history of high uric acid,history of hypertension,and poor dietary habits of all subjects included in the study were statistically analyzed,and it was found that the family history of high uric acid and poor dietary habits were statistically significant(P<0.05).Therefore,the family history of high uric acid and poor dietary habits might be risk factors for the development of chronic uric acid nephropathy.3.Distribution of TCM syndrome types:among the 58 patients enrolled,only 1 had pure deficiency syndrome,belonging to spleen-kidney qi deficiency syndrome;There were 4 cases with pure positive evidence,including 2 cases with cold dampness syndrome and 2 cases with blood stasis syndrome.The remaining 53 cases were all positive with intrinsic false standard.Among the 54 cases of deficiency syndrome,qi and Yin syndrome was the most common,followed by liver and kidney Yin deficiency syndrome,spleen and kidney qi deficiency syndrome,spleen and kidney Yang deficiency syndrome and Yin and Yang deficiency syndrome.Among the 57 patients with standard evidence,the most were dampness-heat syndrome,followed by blood-stasis syndrome,cold-dampness syndrome,and phlegm-turbid syndrome.Among the 53 patients with positive symptoms of this deficiency,the most common are deficiency syndrome of qi and Yin accompanied by dampness and heat,deficiency of liver and kidney Yin accompanied by blood stasis to block collaterals.4.The analysis of TCM syndrome types and oxidative stress indicators of the research object showed that the concentration of superoxide dismutase in the standard demonstration was significantly lower than the normal reference value,and the sequence of concentration from high to low was blood stasis blocking collateral syndrome<cold and damp bi blocking syndrome<phlegm and turbid internal blocking syndrome<damp-heat intrinsic syndrome,and the difference was statistically significant after statistical analysis(P<0.05).Pairing-comparison between different syndrome types showed that the damp-heat syndrome was statistically significant compared with cold-dampness-dampness bi syndrome and blood stasis luo syndrome(P<0.05),indicating that SOD may be related to the standard evidence in the syndrome differentiation and classification of chronic uric acid nephropathy.high to low in turn is the Yin and Yang two gas Yin deficiency syndrome>>spleen deficiency syndrome of kidney Yang deficiency type>liver and kidney Yin deficiency syndrome>spleen and kidney deficiency syndrome,two comparison among different syndrome group,spleen and kidney deficiency and Yin deficiency syndrome is compared between group have significant differences(P<0.05),indicating that the blood uric acid and the 24 h urine protein quantitative may be related to the deficiency syndrome of chronic uric acid nephropathy parting.The other indicators showed no statistical significance after statistical analysis(P>,0.05)Conclusion:The syndrome differentiation and classification of chronic uric acid nephropathy have some correlation with the clinical objective indexes of modern medicine,which can assist the clinical syndrome differentiation and treatment.
