| Objectives:The serum vitamin E(Vitamin E,VE)levels of 163 preterm infants were analyzed to understand the VE levels of premature infants with different genders,number of births,gestational age,birth weight,the relation between birth weight and gestational age,and the VE levels of infected and non-infected preterm infants,to provide a theoretical basis for the early prevention and control of VE deficiency in premature infants.Methods:From January 2019 to August 2020,163 premature infants treated in the department of neonatology of the second affiliated hospital of kunming medical university were selected as the subjects,and were grouped according to gender,number of births,gestational age,birth weight,the relation between birth weight and gestational age and infection.The whole blood samples were collected within 24 hours for 2ml,and the serum was obtained by centrifugation within 24 hours in 0-4℃refrigerated and light-free environment.High performance liquid chromatography(HPLC)was used to determine the VE level.Use SPSS 26.0 statistical software to analyze and compare the differences in VE levels between groups.Results:There were 163 premature infants in this study.The results were as follows:1.Analysis of basic data of preterm infants included in the study:among 163 premature infants,the serum VE data showed a skewed distribution,and the overall level of VE was at[3.5(2.8,4.7)]mg/L.Of these,there were 29 normal cases(17.8%)and 134 cases were lacking(82.2%).Overall VE deficiency rate reached 82.2%.2.Relationship between serum VE level and gender in preterm infants:there were 75 male infants,and the VE level was at[3.60(2.60,4.60)]mg/L.Of these,10 cases were normal(13.3%),and 65 cases were lacking(86.7%);there were 88 female infants,and the VE level was at[3.40(2.80,4.80)]mg/L.Of these,19 cases were normal(21.6%),and 69 cases were lacking(78.4%).The difference was not statistically significant(p>0.05),indicating that the VE level of premature infants may not be related to gender.3.Relationship between serum VE level and fetal number of preterm infants:there were 102 cases in one-child group,and the VE level was[3.90(2.975,495)]mg/L.Of these,25 cases were normal(24.5%),and 77 cases were lacking(75.5%);there were 61 cases in two-child group,and the VE level is[2.40(3.10,3.75)]mg/L,Of these,4 cases were normal(6.6%),and 57 cases were lacking(93.4%).Differences are statistically significant(p<0.05),it can be considered that the serum VE level of single premature infants is significantly higher than that of twin premature infants.4.Relationship between serum VE level and gestational age of preterm infants:the early preterm birth group were 8 cases,and the VE level was[3.05(1.825,4.50)]mg/L.Of these,1 case was normal(12.5%),and 7 cases were lacking(87.5%);there were 16 cases in the preterm birth group,and the VE level was[3.05(2.275,4.65)]mg/L.Of these,1 case was normal(6.2%),and 15 cases were lacking(93.8%);there were 139 cases in advanced preterm infants,and the VE level is[3.50(2.80,4.70)]mg/L.Of these,27 cases were normal(19.4%),and 112 cases were lacking(80.6%).Using Spearman correlation analysis to analyze serum VE levels and gestational age of premature infants,the difference was statistically significant(p<0.05,r=0.174),it can be considered that it was positively correlated between serum VE level and gestational age of premature infants.Further comparison between three samples,the results were no statistical difference.5.Relationship between serum VE level and birth weight of preterm infants:there were 10 cases in the extremely low birth weight group,and the VE level was[3.35(2.075,4.05)]mg/L.Of these,there was no normal case(0.0%),and 10 cases were lacking(100%);There were 93 cases in low birth weight group,and the VE level was[3.30(2.65,4.40)]mg/L.Of these,8 cases were normal(8.6%),and 85 cases were lacking(91.4%);There were 60 cases in normal birth weight group,and the VE level is[4.20(3.20,5.55)]mg/L.Of these,21 cases were normal(35.0%),and 39 cases were lacking(65.0%).Using Spearman correlation analysis to analyze serum VE levels and birth weight of preterm infants,the difference was statistically significant(P<0.05,r=0.255),it can be considered that serum VE concentration in premature infants was positively correlated with birth weight.The serum VE level of premature infants in low birth weight group and normal birth weight group was statistically significant(p<0.05).Three groups of samples were further compared between groups,the result showed that the serum VE level of premature infants with low birth weight[3.30(2.65,4.40)]mg/L lower than that of premature infants with normal birth weight[4.20(3.20,5.55)]mg/L.There was no significant difference in the serum VE level of premature infants in the very low birth weight group and the low birth weight group and the normal birth weight group(p>0.05).6.Comparison of serum VE levels of preterm infants with different birth weight and gestational age:there were 33 cases in small for gestational age group,and the VE level was[3.20(2.30,4.15)]mg/L.Of these,1 cases were normal(3.0%),and 32 cases were lacking(97.0%);there were 125 cases in appropriate for gestational age group,and the VE level is[3.60(2.80,4.80)]mg/L.Of these,25 cases were normal(20.0%),and 100 cases were lacking(80.0%);There were 5 cases in large for gestational age group,and the VE level is[6.70(3.10,7.00)]mg/L.Of these,3 cases were normal(60.0%),and 2 cases were lacking(40.0%).The difference was statistically significant between groups(p<0.05).Further paired comparisons,there was a difference in the level of VE between the small for gestational age group and the appropriate for gestational age group(p<0.05),and it can be considered that the serum VE level of small for gestational age group[3.2(2.3,4.15)]mg/L was lower than that of appropriate for gestational age group[3.6(2.8,4.8)]mg/L;There was a difference in the level of VE between the group with small for gestational age and large for gestational age(p<0.05),and it can be considered that the serum VE level of small for gestational age group 3.2[2.3,4.15)]mg/L was lower than that of large for gestational age group[6.7(3.1,7.0)]mg/L.There was no difference in serum VE between the appropriate for gestational age group and the large for gestational age group(p>0.05).7.Relationship between serum VE levels and infection in preterm infants:the infectious disease group had 140 cases,and the VE level is[3.40(2.725,4.4 0)]mg/L.Of these,17 cases were normal(12.1%),and 123 cases were lacking(87.9%);there were 23 cases in non-infectious diseases group,and the VE level is[5.10(3.10,5.80)]mg/L.Of these,12 cases were normal(52.2%),and 11 cases were lacking(47.8%).The difference was statistically significant(p=0.003),it indicated that the serum VE level of premature infants in infectious disease group was lower than that in non-infectious disease group.Conclusion:1.The total serum VE deficiency rate of 163 premature infants was 82.2%;2.There was no gender difference in serum VE levels in preterm infants,but serum VE level of premature infants was positively correlated with gestational age and birth weight;And VE levels in single preterm infants were higher than twins preterm infants;3.The level of serum VE in small for gestational age preterm infants were less than appropriate for gestational age preterm infants and large for gestational age preterm infants;4.The serum VE level of premature infants was related to infection.The serum VE level of premature infants with infectious diseases is lower than that of premature infants without infectious diseases.A low level of vitamin E is one of the factors leading to infection in premature infants. |
