Clinical Efficacy Of Xi Bi Tong Fang In Treating Early Knee Osteoarthritis Of Liver And Kidney Deficiency Type And Its Effect On Serum MMP-13

[Objective]The purpose of this study was to observe the differences in serum MMP-13 level,knee VAS score,WOMAC score and TCM symptom grading quantitative score between the two groups before and after treatment by comparing the oral group of Xi Bi Tong Fang with the oral group of conventional western medicine(glucosamine sulfate capsule and diclofenac sodium sustained release tablet).In order to further explore the relevant mechanism of Xi Bi Tong Fang in the treatment of liver and kidney deficiency type of early knee osteoarthritis,provide theoretical guidance for the treatment of liver and kidney deficiency type of early knee osteoarthritis,and promote the clinical application and promotion of this prescription.[Methods]A total of 100 patients with early knee osteoarthritis with deficiency of liver and kidney in our hospital were selected as the research objects.All the patients were divided into control group and experimental group with 50 cases in each group according to random number table method.The experimental group was given Xi Bi Tong Fang orally,and the control group was given Glucosamine Sulfate Capsules+Diclofenac Sodium Sustained Release Tablets orally.After 4 weeks of treatment,the serum MMP-13 level,WOMAC score,VAS score and TCM symptom grading quantitative score were compared between the two groups before treatment,at the end of treatment,2 weeks after treatment and 4 weeks after treatment.[Results]Before treatment,there was no statistical significance in VAS score,WOMAC score and TCM symptom grading quantitative score between 2 groups(P>0.05).After treatment,VAS score,WOMAC total score and TCM symptom grading quantitative score of the two groups were significantly lower than before treatment,and there were significant differences between groups(P<0.05),and the experimental group was better than the control group.In WOMAC sub-item score,the comparison of pain rating between groups was statistically significant(P<0.05).In the joint function scores,the comparison between groups at 4 weeks after treatment showed statistical significance(P<0.05),while there was no statistical significance at other time points(P>0.05).In the rigidity scores,the comparison between the two groups showed statistical significance at the end of the treatment(P<0.05),while the remaining time points showed no statistical significance(P>0.05).Before treatment,there was no significant difference in serum MMP-13 between 2 groups(P>0.05).After treatment,serum MMP-13 was significantly decreased in both groups,and there was a significant difference between the two groups(P<0.05).The change degree of MMP-13 level in experimental group was more obvious than that in control group.[Conclusion]This study shows that Xi Bi Tong Fang has a clear effect in the treatment of early knee osteoarthritis with deficiency of liver and kidney,and can effectively relieve the clinical symptoms of patients with early knee osteoarthritis and improve the function of knee joint.It can also reduce the expression level of MMP-13 in serum of patients with KOA,which may significantly inhibit the expression of MMP-13.By inhibiting the expression of MMPs,it can reduce the degradation of extracellular matrix and protect the articular cartilage,so as to effectively prevent the progression of osteoarthritis.The clinical efficacy of this formula is safe and effective,without obvious side effects,combined with Chinese medicine dialectical application effect is significant,worthy of clinical promotion and application.
