Comparative Evaluation Of(F/T)/PSAD And %(F/T)/PV In Initial Diagnosis Of Prostate Cancer | | Posted on:2021-12-11 | Degree:Master | Type:Thesis | | Country:China | Candidate:F Q Zhang | Full Text:PDF | | GTID:2504306470973449 | Subject:Clinical Medicine | | Abstract/Summary: | PDF Full Text Request | | Objective: Comparative evaluation of percentage free prostate-specific antigen/prostate-specific antigen density-(F/T)/PSAD and percentage free prostate-specific antigen density percentage-%(F/T)/PV in patient with initial diagnosis of prostate cancer.Method:From January 2018 to January2019,we collected the serum PSA and f PSA、transrectal ultrasound results and pathological results of 145 patients with prostatic related diseases,who were initially diagnosed by transrectal ultrasound guided prostate biopsy and/or transurethral resection of prostatic surgery in the urology department of the first center clinical hospital of Tianjin Medical University.We had calculated prostate volume,percentage free prostate-specific antigen,prostate antigen density,percentage free prostate-specific antigen/prostate-specific antigen density-(F/T)/PSAD and percentage free prostate-specific antigen density percentage-%(F/T)/PV.We analyzed the dates by SPSS software(23.0version)and Medcalc software.The quantitative datas were compared by t-test,tested the relationship between the indexes and the detection rate of prostate cancer by Spearman correlation analysis.According to the correlation analysis results,the receiver operating characteristic curve(ROC)of the subjects with relevant parameters was analyzed,and Logistic regression analysis was used to analyze the relationship between the related parameters and the diagnosis of prostate cancer.Result:When PSA was between(4-20)ng/ml,(F/T)/PSAD had a certain diagnostic advantage,but inferior to F/TPSA;when PSA was more than 20ng/ml,it had a certain diagnostic advantage,but inferior to PSAD.When serum PSA was between(4-10)ng/ml,even(4-20)ng/ml,F/TPSA had obvious diagnostic advantage in the diagnosis of prostate cancer,which was obviously better than PSAD.Only when the serum PSA was more than 20ng/ml,%(F/T)/PV had a certain statistical significance,but had no better advantage.(F/T)/PSAD had a negative correlation with the detection rate of prostate cancer,when PV was less than80 ml.The AUC value was higher than that of PSA and PSAD.Binary logistic regression showed that(F/T)/PSAD had a certain research valuewhen PV was less than 80 ml.But when PV>80ml,(F/T)/PSAD had no research value.%(F/T)/PV had no correlation with prostate cancer detection rate in different PV intervals.Conclusion: 1.(F/T)/PSAD had a certain value in the diagnosis of prostate cancer.When(F/T)/PSAD was less than1.01,we should be alert to the possibility of prostate cancer.2.When serum PSA was less than 4ng/ml,(F/T)/PSAD had no diagnostic value.When serum PSA was between(4-20)ng/ml,(F/T)/PSAD had a negative correlation with prostate cancer detection rate,but F/TPSA should be taken as the only reference factor to decide whether to carry out prostate biopsy.3.When PV<35 、 35-50 、 50-80 ml,(F/T)/PSAD was negatively correlated with the detection rate of prostate cancer,especially when PV was between 35-50 and50-80 ml,(F/T)/PSAD had a obvious value in the diagnosis of prostate cancer.For patients with PV<80ml,PSAD should be mainly referred to decide whether to carry out prostate biopsy.(F/T)/PSAD could be played as a auxiliary reference,and its clinical value is small.4.Only when serum PSA>20ng/ml,%(F/T)/PV was correlated with prostate cancer detection rate,but its AUC value was significantly lower than PSA 、 PSAD and(F/T)/PSAD.%(F/T)/PV had no obvious clinical application value. | | Keywords/Search Tags: | PSA, F/TPSA, PSAD, PCa, Diagnosis | PDF Full Text Request | Related items |
