Objective: In order to detect H.pylori genotypes in gastric mucosal biopsy of patients with Chronic non-atrophic gastritis(CNAG),Chronic atrophic gastritis(CAG),and Gastric cancer(GC)combined with Helicobacter pylori(H.pylori)in Qinghai region,The genotyping differences of H.pylori among different diseases were compared.The association between each group of diseases and H.pylori genotypes was analyzed,and the influence of H.pylori gene polymorphism on the outcome of associated diseases was preliminarily discussed.Methods: A collection of 162 patients who met the inclusion criteria in the Affiliated Hospital of Qinghai University from December 2019 to December 2020 were collected.The 13 C or 14 C urea breath test confirmed the presence of H.pylori infection by gastroscopy and histopathological examination.A total of 162 patients with CNAG,CAG,and GC were the subjects of study.Gastric mucosal tissue was extracted by gastroscopy and separated and cultured to obtain H.pylori strains.DNA was extracted from the H.pylori strains obtained above,and PCR and sequencing methods were used to detect the content in the samples.Strain genotypes,including CagA,Vac A(s1,s2,m1,m2),Ure A,Ure B,ice A genes,bab A,oip A and other genes,and analyze the relationship between the above-mentioned genotypes and the three groups of diseases,and compare the data Perform statistical analysis.Results: 1.The distribution of H.pylori genotypes and the gender and age differences among patients in the three groups were analyzed,and the results indicated that there was no significant statistical difference between H.pylori genotypes and patients’ gender and age(P > 0.05).2.A total of 162 gastric mucosa samples of H.pylori infection were collected in this study,and 78 of them were successfully sequenced.A total of 72 cases of CagA gene were detected,and the overall detection rate was 92.31%(P>0.05).The positive rates of Bab A,Oip A,Ure B were 64.1%(P>0.05),23.08%,64.10%,79.49%(P >0.05),respectively.The positive rates of Vac A s1,Vac A s2,Vac A m1 and Vac A m2 subtypes were 75.64%,15.38%,42.31% and 53.85%(P>0.05),respectively.There was no statistical difference in the distribution of H.pylori genotypes among the three groups(P>0.05).3.Vac A s1/ m2 is the most common subtype combination of Vac A genes,and it has the highest infection rate in Vac A genotypes.Conclusions: 1.CagA,Vac A and urease genes were the dominant genotypes in H.pylori infected population in Qinghai,and there were multiple mixed infections of H.pylori in Qing Hai.Vac A gene has a variety of alleles,s1 and m2 are the most common subtypes.2.There were regional differences in the distribution of H.pylori genotypes,but no correlation was found with the pathological types of the disease.3.The genotypes of H.pylori could not be used as a specific marker to determine whether chronic gastritis could develop into gastric cancer and precancerous lesions.
