Clinical Efficacy Evaluation Of Modified Huanglian Wendan Decoction In Treatment Of Metabolic Syndrome And Its Effect On Serum CHEMERIN,IL-6 And TNF-Α

Objective:this study by observing the rhizoma coptidis Wen Dan tonga subtraction side treatment phlegm turbidity resistance type the clinical curative effect of metabolic syndrome(MS)and serum of patients with chemotactic factor(Chemerin),interleukin6(IL-6),tumor necrosis factor alpha(TNF alpha)level,the influence of explore the mechanism of action of the treatment of MS,providing a new method for traditional Chinese medicine treatment of MS.Methods:Sixty-two patients who met the diagnostic requirements of MS with internal obstruction of phlegm and turbidity were collected and randomly divided into treatment group and control group,with 31 patients in each group.The control group received Western medicine comprehensive intervention treatment(including diet control,exercise intervention,routine hypoglycemic,lipid regulation and antihypertensive treatment);The treatment group was additionally treated with Huanglian Wendan Decoction(200ml,decocted in water,one dose per day)on the basis of the control group.Patients in both groups were followed up for 12 weeks(two courses of treatment).The changes of BMI,WC,FPG,2HPG,Hb A1 c,TG,TC,LDL-C,HDL-C,SBP,DBP,serum Chemerin,IL-6,TNF-α,HOMA-IR,ISI and TCM syndrome scores in 2 groups were compared before and after treatment,and the safety indexes were tested.Finally,SPSS21.0 software is used to analyze and process the data.Results:1.Comparison of overweight(or obesity)indexes: After treatment,BMI and WC of the two groups were significantly decreased(P<0.01),and the treatment effect of the treatment group was significantly better than the control group(P<0.05).2.Comparison of blood glucose indexes: FPG,2HPG and Hb A1 c in the two groups were significantly decreased after treatment(P<0.01),and the reduction effect of FPG,2HPG and Hb A1 c in the treatment group was better than that in the control group(P<0.05).3.Comparison of blood lipid indexes: After treatment,TG,TC and LDL-C in the two groups were significantly decreased(P<0.01),while HDL-C in the two groups were significantly increased(P<0.01).The reduction of TG,TC,LDL-C and the increase of HDL-C in the treatment group were better than those in the control group(P<0.05).4.Comparison of blood pressure levels: SBP and DBP in the two groups were significantly lower after treatment than before treatment(P<0.01),and SBP and DBP in the treatment group were more significantly lower than those in the control group(P<0.05).5.Comparison of serum levels of Chemerin,IL-6 and TNF-α: After treatment,serum levels of Chemerin,IL-6 and TNF-α in the two groups were significantly lower than before treatment(P<0.01),and the therapeutic effect of the treatment group was significantly better than that of the control group(P<0.01).6.Comparison of HOMA-IR and ISI: After treatment,HOMA-IR decreased in both groups compared with before(P<0.01),while ISI increased in both groups compared with before(P<0.01).The decrease of HOMA-IR and the increase of ISI in the treatment group were more significant than those in the control group(P<0.05).7.Comparison of TCM syndrome scores and clinical efficacy: After treatment,the TCM syndrome scores of both groups decreased(P<0.01),especially the treatment group had better effect than the control group(P<0.01).The total effective rate of the treatment group was 100%,and the total effective rate of the control group was 93.5%,which was significantly better than the control group(P<0.01).8.Comparison of safety indicators: there were no adverse symptoms and signs related to the disease in the two groups before and after treatment.Conclusion: Rhizoma coptidis Wen Dan tonga subtraction side treatment phlegm turbidity resistance type MS patients has obvious clinical efficacy,compared with the control group patients will be better able to reduce weight,regulate glucolipid metabolism,reduce blood pressure patients,its mechanism may be through the lower serum Chemerin,IL-6,the level of TNF alpha,reduce the body’s inflammatory response,improve the insulin resistance,so as to achieve the purpose of the treatment of metabolic syndrome components.Many years of clinical studies have proved that Huanglian Wendan Decoction is safe and effective and worthy of being widely used in the treatment of metabolic syndrome.
