| Objective:Observe the clinical efficacy of pemphigus granule on bullous pemphigoid with shi re du yun type accumulation and its influence on hematological indexes.Through network pharmacology and molecular docking technology,discover and verify the material basis and mechanism of action of tian pao chuang granules in the treatment of BP,and provide new scientific evidence for clinical rational use of drugs and modernization of traditional Chinese medicine.Methods:1.A total of 169 cases of BP patients with shi re du yun type accumulation were selected from January 2015 to December 2019 in our hospital(affiliated Hospital of Tianjin Academy of Traditional Chinese Medicine),the data of sex,age,onset season,length of stay,age of onset and past history were collected.2.Analyze the correlation between BPDAI and hemagglutination test,D-Dimer,overall immune,inflammatory indexes and other data.3.According to the different methods of treatment,169 patients with BP can be divided into exposure group and non exposed group and non exposed group only western medicine routine treatment,exposure group therapy for western medicine routine treatment combined tian pao chuang particles,comparing the difference between the liver and kidney function index between two groups.4.Analyze differences in hospitalization time,laboratory indicators,and TCM syndrome scores between the two groups.1.Screening the active components and targets of pemphigus granules;Determine BP related targets;2.Cytoscape 3.7.2 software was used to draw the network diagram of "drug-component-target-disease";To construct the protein interaction network map;3.Connect the top 3 disease targets in the protein interaction network diagram with the top 6core components of the pemphigus granules in the "drug-component-target-disease" network diagram with the number of regulatory targets in the network diagram,and carry out molecular docking Verify and construct a schematic diagram of the best connection between the core components.Result:1.Equilibrium test of baseline data between the two cohorts: A total of 169 cases were collected,including 108 cases in the exposed group and 61 cases in the non-exposed group.Baseline data such as age,gender and pre-treatment BPDAI score of patients in the two cohorts showed no statistical difference and were comparable.A total of 169 cases were collected in this study,including 97 males and 72 females,with a male-to-female ratio of1.35:1.137 cases(81.07%)were hospitalized for the first time,and 32 cases(18.93%)were diagnosed with recurrence or aggravation.Seasonal distribution: 38 cases in spring,44 cases in summer,46 cases in autumn and 41 cases in winter,P > 0.05.2.137 cases had a history of one or more other diseases,accounting for 81.1% of the total;80 cases of hypertension,22 cases of coronary heart disease,12 cases of cerebral infarction,3 cases of cerebral hemorrhage,24 cases of diabetes;3.D-Dimer and FIB indexes were positively correlated with BPDAI scores.4.Safety indicators: There was no statistical significance in the changes of related indicators between the two groups before and after treatment(P > 0.05).There are 12 intersecting targets between tian paochuang granule regulatory targets and BP-related targets.The top 6 regulated targets are quercetin,kaempferol,luteolin,β-carotene,and wood.Butterfalin A,wogonin.It shows that these compounds play a major role in the treatment of BP with tian pao chuang granules.The current number of targets regulated by compounds is B-cell lymphoma 2(BCL-2),interleukin 6(IL-6),intercellular cell adhesion molecule 1(ICAM-1),vascular cell Vascular cell adhesion molecule 1(VCAM-1)is the main target of pemphigus granules in the treatment of BP.Conclusion:1.BP patients have a long course of disease and are prone to recurrence.Male patients are more than female patients,and the clinical incidence is more than 60 years old.There is no obvious correlation between the incidence and season,and there is no significant difference between the four seasons.2.Most patients are complicated with internal diseases,so attention should be paid to the prevention and treatment of cardiovascular and cerebrovascular diseases to improve the survival rate of patients.3.BP patients have changes in D-Dimer and FIB indicators;4.Tian pao chuang granules are effective in the treatment of shi re du yun type BP and worthy of clinical promotion.1.The main targets of pemphigus granules in the treatment of BP are BCL-2,IL-6,ICAM-1,and VCAM-1;2.The compounds that play a major role in the treatment of BP with tian pao chuang granules are quercetin,kaempferol,luteolin,β-carotene,oroxylin A,and wogonin;3.The therapeutic effect of pemphigus granules on BP is mainly reflected in the regulation of immune function and inflammatory state. |
