| PurposeThis paper mainly discusses the relationship between dietary fiber(DF)and inflammatory bowel disease(IBD)from two aspects: the relationship between DF and the risk of IBD,and the difference of DF intake between patients with IBD in remission or mild to moderate active period and the general population,so as to provide the basis for the prevention of IBD and the guidance of DF intake in IBD patients.MethodsTwo investigators independently searched Pubmed、Web of Science、Ovid、CNKI and VIP for all reported studies,the relationship between DF intake and the risk of IBD,and the difference of DF intake between patients during remission or mild to moderate activity of IBD and healthy control group were assessed by meta-analysis with stata15.0 software.Results1.Meta-analysis of DF and risk of IBD:A total of 11 articles were extracted,including 4 cohort studies and 7 case-control studies.Among them,10 studies explored the relationship between DF intake and ulcerative colitis,and 9 studies explored the relationship between DF intake and Crohn’s disease,enrolling 2408 patients.The results of Meta analysis showed that DF intake was not related to the occurrence of ulcerative colitis RRs = 0.88,95% CI(0.72,1.07),P = 0.196.However,DF intake had protective effect on the occurrence of Crohn’s disease RRs = 0.53,95% CI(0.37,0.77),P = 0.001,the results were statistically significant.2.Meta-analysis of DF intake during remission or mild to moderate activity of IBD:A total of 7 clinical studies were extracted,including 1cohort study,2 cross-sectional studies and 4 case-control studies.Compared with the general population,DF intake of IBD patients was significantly reduced,SMD=-0.32,95%CI(-0.60,-0.04),P=0.027.Subgroup analysis was carried out for the included disease types,the results showed that the DF intake of patients with ulcerative colitis and Crohn disease in remission or mild to moderate activity period was significantly lower than the general population,the combined effects were SMD=-0.61,95%CI(-0.91~-0.31),P=0.000 and SMD=-0.27,95% CI(-0.41~-0.12),P=0.000 respectively.ConclusionDietary fiber intake is not related to the risk of ulcerative colitis,but it is negatively related to the risk of Crohn’s disease.Appropriate increase dietary fiber intake may help to prevent the occurrence of Crohn’s disease.At the same time,the meta-analysis also showed that the dietary fiber intake of IBD patients in remission or mild to moderate activity period was significantly lower than that of the general population. |
