Objectives To explore the effect of nano carbon(Carbon Nanoparticles Suspension Injection,CNSI)that was used to the patient at c N0 stage of papillary thyroid microcarcinoma when to be carried out prophylatic central compartment neck dissection,it is the application value and significance of nano-carbon.Methods It was divided to two groups randomly that the patients went to a docor in our hospital during Jun 2017 to May 2019.Parathyroid hormone,laryngoscopy and others were recorded before and after the operation.The operation should be carried out according to the management guidelines of diagnosis and treatment.The tracer group used CNSI in prophylatic central compartment neck dissection when the primary tumor was excised,the control group was not applied CNSI.After the operation,the patients were given calcium supplements by IVT,routinely;According to p TNM stage and assessment of structural disease recurrence risk,When suppressing TSH with Levothyroxine sodium tablets after operation,Measuring TSH,Parathyroid hormone and Serum calcium regularly,the patient of disodered pronunciation and fixed cord would be repeated laryngoscopy termly.The follow-up of patient was no less than 6 months.Results The difference was not significant between the two groups in age,gender,maximum diameter of primary tumor,extrathyroidal extension and multifocal disease(P>0.05).The mean of lymph nodes was 10.67±2.45 in tracer group by prophylatic central compartment neck dissection;the mean was 4.22±1.64 in control group.The difference was statistically significant(P<0.05).The mean of lymph nodes was 7.00(6.00,8.00)with diameter less than 0.2cm in tracer group,the mean was 0.00(0.00,1.00)in control group,there was significant difference between the two group(P<0.01).The median of positive lymph nodes was 3.00(0.00,6.00)in tracer group,the median was 0.00(0.00,2.00)in control group,The difference was statistically significant(P<0.05).The total of parathyroid transplantation were 46,the control group was 35,the difference was statistically significant(P<0.05).The quantity of large-volume(quantity of positive lymph nodes>5)lymph nodes metastases were 17 in tracer group,the control group was 0,There was significant difference(P<0.05).The total of complication was 4 in tracer group,including permanent hypoparathyroidism,postoperative bleeding,incision infection and temporary dysphonia,the control group was 2,there was no significant difference(P>0.05).Conclusions It is help to find more lymph nodes by using Carbon Nanoparticles Suspension Injection in prophylatic central compartment neck dissection at c N0 stage of papillary thyroid microcarcinoma patients,especially the metastatic lymph nodes and the diameter of lymph node less than 0.2cm,it is used to identify large-volume lymph node metastasis.it is helpful to avoid accidental parathyroidectomy and transplant parathyroid glands.There is not influence on the total incidence of complications.Figure 12;Table 11;Reference 125...
