Objective: To investigate whether helicobacter pylori infection colonized in human larynx is a risk factor for laryngeal cancer.Whether Helicobacter pylori promotes laryngeal cancer by affecting the expression of C-myc,CyclinD1 and P16 genes in patients.To provide a new experimental basis,theoretical ideas and feasible strategies for the prevention and treatment of laryngeal cancer.Methods: 63 cases of laryngeal carcinoma patients diagnosed in the First Affiliated Hospital of Yangtze University Otolaryngology Head and Neck Surgery from January2017 to February 2021.Immunoin-situ hybridization was used to detect HPV type 1,6,11,16,18 and 31 in the laryngeal tumor tissues of 63 patients with confirmed laryngeal cancer.40 patients with laryngeal cancer were included in the experimental group after exclusion of HPV positive laryngeal cancer tissue,and 40 patients with vocal cord polyps clinically and pathologically confirmed in hospital during the same period were selected as the control group.All enrolled patients were selected to collect complete clinical information and the paraffin specimens including laryngeal cancer tissue,tissue and vocal cord polyp tissue.Immunohistochemistry was used to detect the Helicobacter pylori and the expression of C-myc,CyclinD1 and P16 protein in the laryngeal tissues of 40 patients diagnosed with laryngeal cancer.The infection of Helicobacter pylori in the laryngeal tissue of 40 patients with confirmed vocal polyp was detected by immunohistochemical method.The chi-square test was used to analyze the differences of Helicobacter pylori infection between laryngeal malignant tumor tissue and vocal cord polyp tissue.To investigate whether Helicobacter pylori infection is a carcinogenic factor of laryngeal cancer,and to analyze the relationship between Helicobacter pylori infection,c-Myc,CyclinD1 and P16 proteins and the degree of pathological differentiation,clinical stage,T stage,invasion and metastasis of laryngeal cancer.Pearson test was used to analyze the relationship between Helicobacter pylori,C-myc,CyclinD1,P16 with the clinical parameters and clinicopathological features.The association between Helicobacter pylori infection and the expression of C-myc,CyclinD1 and P16 protein in laryngeal carcinoma was analyzed using contingency coefficient analysis.Results:(1)The positive rate of Helicobacter pylori infection in laryngeal cancer tissues was higher than that in vocal cord polyp tissues(χ~2=11.25,(49)<0.05).(2)Histological grade of the tumor: the positive rate of Helicobacter pylori infection in low-differentiated laryngeal cancer tissues was higher than that in high-middle differentiated laryngeal cancer tissues(χ~2=5.119,(49)<0.05).The positive expression rate of C-myc protein in poorly differentiated laryngeal cancer tissues was higher than that in high-middle differentiated laryngeal cancer tissues(χ~2=6.253,(49)<0.05)and the positive expression rate of CyclinD1 protein in poorly differentiated laryngeal cancer tissues was higher than that in high-middle differentiated laryngeal cancer tissues(χ~2=4.405,(49)<0.05),while the positive expression rate of P16 protein in poorly differentiated laryngeal cancer tissues was lower than that in high-middle differentiated laryngeal cancer tissues(χ~2=4.269,(49)<0.05).T stage: the positive rate of Helicobacter pylori infection in laryngeal cancer at T3+T4 stage was higher than that at T1+T2 stage(χ~2=5.871,(49)<0.05).The positive epression rate of C-myc protein in lryngeal cancer tissues at T3+T4 stage was higher than that at T1+T2 stage(χ~2=4.945,(49)<0.05)and the positive expression rate of CyclinD1 protein in lryngeal cancer tissues at T3+T4 stage was higher than that at T1+T2 stage(χ~2=3.872,(49)<0.05),whie the positive expression rate of P16 protein in cancer tissues at T3+T4 stage was lower than that at T1+T2 stage(χ~2=8.087,(49)<0.05).Clinical stage: The positive rate of Helicobacter pylori infection in advanced laryngeal cancer(Ⅲ+Ⅳ)was higher than that in early laryngeal cancer(Ⅰ + Ⅱ)(χ 2=8.210,(49)< 0.05).The positive expression rate of C-myc protein in advanced laryngeal cancer tissues(Ⅲ+Ⅳ)was higher than that in early laryngeal cancer tissues(Ⅰ+Ⅱ)(χ~2=6.087,(49)<0.05)and the positive expression rate of CyclinD1 protein in advanced laryngeal cancer tissues(Ⅲ+Ⅳ)was higher than that in early laryngeal cancer tissues(Ⅰ+Ⅱ)(χ~2=4.713,(49)<0.05),while the positive expression rate of P16 protein in advanced laryngeal cancer tissues(Ⅲ + Ⅳ)was lower than that in early laryngeal cancer tissues(Ⅰ + Ⅱ)(χ~2=4.000,(49)<0.05).Metastasis: The positive rate of Helicobacter pylori infection in laryngeal cancer patients with distant or lymphatic metastasis was higher than that in laryngeal cancer patients without distant or lymphatic metastasis(χ 2=9.920,(49)<0.05).The ositive rate of C-myc protein in laryngeal cancerpatients with distant or lymphatic metastasis was higher than that in laryngeal cancer patients without distant or lymphatic metastasis(χ~2=6.313,(49)<0.05)and the positive rate of CyclinD1 protein in laryngeal cancerpatients with distant or lymphatic metastasis was higher than that in laryngeal cancer patients without distant or lymphatic metastasis(χ~2=7.305,(49)<0.05),while the positiverate of P16 in laryngeal cancer patients with distant or lymphatic metastasis was lower than that in laryngeal cancer patients without distant or lymphatic metastasis(χ~2=3.889,(49)<0.05).(3)There was a positive correlation between Helicobacter pylori infection and C-myc protein expression(χ~2=4.062,r =0.319,(49)<0.05)and there was a positive correlation between Helicobacter pylori infection and CyclinD1 protein expression(χ~2=4.477,r=0.335,(49)< 0.05),however there was a negative correlation between Helicobacter pylori infection and P16 protein expression in laryngeal cancer tissues(χ~2=5.934,r=-0.385,(49)<0.05).Conclusion: 1.The infection rate of Helicobacter pylori in laryngeal cancer was higher than that in vocal cord polyps.Compared with Helicobacter pylori negative laryngeal cancer,Helicobacter pylori positive laryngeal cancer has poor histopathological differentiation,late clinical stage and high metastasis rate.2.The positive expression rate of C-myc and CyclinD1 protein was higher in poorly differentiated,advanced and metastatic laryngeal cancer tissues,while P16 protein was lower poorly differentiated,advanced and metastatic laryngeal cancer tissues.3.Helicobacter pylori infection was positively correlated with the positive expression of C-myc and CyclinD1 protein,while Helicobacter pylori infection was negatively correlated with the positive expression of P16 protein,suggesting that the high expression of C-myc and CyclinD1 protein and the low expression of P16 protein may be related to the poor histopathological differentiation,late clinical stage and high metastasis rate of Helicobacter pylori positive laryngeal cancer,but it still needs to be further verified by numerous basic experiments.
