Analysis Of Influencing Factors Of Helicobacter Pylori Infection In Children With Recurrent Abdominal Pain

Abstract/Summary:
Objective: To understand the status of Helicobacter pylori infection in children with recurrent abdominal pain,and analyze the influencing factors of Helicobacter pylori infection in children with recurrent abdominal pain,so as to provide reference basis for the prevention of Helicobacter pylori infection in children in this area.Methods: A total of 175 children with recurrent abdominal pain aged 5 ~ 14 who received HP test according to their condition from July 1,2020 to December 31,2021 in pediatrics of the Affiliated Hospital of Yan’an University were selected,including 85 male children and 90 female children.All the children underwent 13 C urea breath test,and some children underwent gastroscopic histological examination to further determine whether Hp infection existed.The influencing factors were analyzed by questionnaire.After obtaining the informed consent of the family members,a questionnaire survey was conducted to collect the general information,previous gastrointestinal diseases,family and family economic situation,eating habits,living habits,family members and the knowledge of the family members about Hp.The χ~2 test was used for univariate analysis of independent variables,and the independent variables with statistically significant difference in univariate analysis were further analyzed by binary logistic regression.Result:1.A total of 175 children with recurrent abdominal pain were collected in this study,including 65 children with Hp infection and 110 children without Hp infection,with an infection rate of 37.14%.2.The infection rate of Hp in male children was 37.65%,and that in female children was 36.67%,with no statistical significance(P > 0.05).3.The infection rate of Hp was different in children with recurrent abdominal pain in different age groups.The infection rate of children aged 5 to 7 years was 17.54%,the infection rate of children aged 8 to 10 years was 37.50%,and the infection rate of children aged 11 to 14 years was 57.41%.Hp infection rates increase with age.4.Univariate analysis showed that Hp infection in children with recurrent abdominal pain was related to age,place of residence,previous history of digestive tract such as chronic gastritis / peptic ulcer,family monthly income,mother’s educational level,frequency of eating fruit every week,drinking raw water,sharing toothbrush cup with family,sharing tableware water cup with family,whether they often wash their hands before and after meals,parents’ health habits,parents’ history of stomach disease and other factors,The difference was statistically significant(P < 0.05).The factors with statistically significant difference in univariate analysis were included in binary logistic regression for further analysis.The results showed that eating fruit ≥4 times/week(OR=0.245,95%CI: 0.086-0.702)was a protective factor for Hp infection,and age increased(OR=2.103,95%CI: 1.297-3.410),previous history of digestive diseases such as chronic gastritis/peptic ulcer(OR=3.019,95%CI: 1.253-7.273),frequent drinking of raw water(OR=3.212,95%CI: 1.223-8.435),parents’ history of gastric diseases(OR=2.460,95%CI: 1.180-5.129)is a risk factor for Hp infection.5.The analysis of the awareness of Hp related knowledge of 175 children’s family members showed that 44% of the family members knew little about Hp related knowledge,51% of the family members knew some Hp knowledge,and only 5% of the family members knew Hp knowledge and paid attention to it in life.The awareness rates of family members on Hp common knowledge such as Hp is infectious,the main transmission route of Hp,the susceptible population of Hp,Hp is the carcinogen of gastric cancer,Hp infection can cause chronic gastritis and peptic ulcer were 46.86%,36%,34.29%,28% and 26.29% respectively.Conclusion:1.In this study,the infection rate of Hp in 175 children with recurrent abdominal pain was 37.14%,similar to that reported in previous literatures in China,but higher than that in natural population of children and adolescents in China.2.Hp infection in children with recurrent abdominal pain is not related to gender,but related to age.The infection rate of Hp increases with age.3.Through the analysis of the influencing factors of Hp infection in children with recurrent abdominal pain,it is suggested that the history of chronic gastritis/peptic ulcer,drinking raw water frequently,and parents’ history of gastric disease are the risk factors of Hp infection in children,and eating fruit ≥4 times a week is the protective factor of Hp infection in children.4.In this study,it was found that family members’ knowledge of Hp was low,and the popularization of Hp knowledge in this region should be strengthened.
