Study On The Improvement Of Health Management Ability Of The Elderly With Chronic Diseases By Group Work Intervention

According to data provided by the National Health Commission,more than 180 million elderly people in China have chronic diseases in 2019,and the proportion of people suffering from one or more chronic diseases is up to 75%.Chronic diseases have become one of the main problems affecting the health of the elderly.Due to the characteristics of low cure rate of chronic diseases,long course of disease and relatively stable symptoms of patients,the treatment of chronic diseases requires patients to carry out long-term health management of chronic diseases on the basis of regular medical examination and management.However,at present,the elderly with chronic diseases often suffer from emergencies due to insufficient health management capabilities.Therefore,the use of group work intervention in this study to help the elderly with chronic diseases to improve their health management capabilities has important practical significance.This essay is problem-oriented,using questionnaire surveys and interviews to conduct an in-depth investigation of the elderly with chronic diseases in the CL community,and found that the elderly patients in this community have weak management awareness,single management methods,lack of social support and other issues in the process of chronic disease management.At the same time,it also has corresponding problem solving needs.Based on this,the author guided by the empowerment theory,social support theory,cognitionand behavior theory,and social learning theory,recruited 12 elderly people with chronic diseases to form a group,and carry out six "pay attention to health,enjoy life" chronic elderly health management education group service.The main goal of the group activity is to improve the health management ability of the elderly with chronic diseases.The group plan is designed from the five research perspectives of the client’s cognition,behavior,psychological endurance,skill learning and support,as well as qualitative and quantitative research methods are used to evaluate group services effectiveness.The research results prove that with the assistance of group services,the elderly members of chronic diseases have improved their awareness of chronic disease management,learned health management skills,gradually developed healthier living habits,at the same time,they have reconstructed more abundant,favorable social support network,and their self-health management ability has been improved to a certain extent.The good service results verified the feasibility of group work intervented the practical research of improving the health management ability of the elderly with chronic diseases,and on the basis of this service,the researchers suggest that the health management services for the elderly with chronic diseases can be carried out from three aspects: improving the professional level of social work service,perfecting the community health management system,establishing community health mutual help,and family health advocacy and prevention mechanism,to provide certain experience for future associated practical research in this field.
