The Practical Exploration Of Group Work In The Health Education Of The Elderly Patients With Chronic Diseases

Ageing in the current high-speed development background,economic life,welfare and social resources of the balance of supply and the new old age poverty problems of an ageing population puts forward the new challenge,and cause the attention of the society from all walks of life continue.Especially the elderly physical problems,due to the physiological function and organs over time will be aging with irreversibility,individuals entering old age after the hard to avoid has certain parts or organs functional disorder,followed by a variety of diseases,and the pressure of the disease and lead to changes in the emotional and social characteristics,social progress and improve the elderly services has become one of the important tasks of era.This research with the aid of internship agency project resources,utilizing the method of group work intervention in elderly patients with chronic diseases health education problems,will be in elderly patients with chronic disease crowd gathered to carry out health education,to promote their health knowledge and cultivate healthy idea and behavior,thereby shaping healthy physical and mental environment.In this paper,based on the "central government" project,using the method of questionnaire,interview,and combined with practice the elderly have archived data,on the basic situation of the old people in the project and health status were investigated,sort out the current problems of health education,and to evaluate the actual needs of the elderly health education.According to the needs assessment,the elderly in the project have not only the need for chronic disease health knowledge,but also the need to acquire chronic disease life skills and establish positive health values.Then,on the basis of analyzing the feasibility of the group's work intervention,this paper discusses with the service objects the design of the group's work service scheme,which is implemented by social workers.In practice,six group activities,such as re-understanding the disease,safe medication,reasonable diet and healthy exercise,were used to replace the traditional life concept of the service object.After that,process assessment and effect assessment were used to examine the effects of group activities,and Suggestions on improvement of education were proposed based on the summary and reflection of the whole paper.
