| Diabetes,as one of the main chronic diseases threatening human health,can cause more than one hundred complications.It requires patients to fight for a long time with the disease,and has a high fatality rate and incurable rate.Therefore,patients with diabetes need longterm medication to maintain their health.Being healthy,which means that diabetic patients must carry out self-health management for a long time to control their condition.However,due to their high age,elderly diabetic patients have already developed many bad living habits.They have the characteristics of high prevalence and prone to hypoglycemia.In addition,they lack correct cognition and behavior ability.The self-efficacy of elderly diabetic patients is generally low..The author takes self-efficacy theory as the theoretical basis of this research,applies the self-efficacy improvement group work model to the self-health management of elderly diabetic patients,and relies on the community to carry out self-health management ability improvement group activities for elderly diabetic groups.In the previous investigation and visit,the author used the Diabetes Self-Management Behavior Scale and the SelfManagement Effectiveness Scale to get a preliminary understanding of the basic conditions of 50 elderly diabetic patients in the G community.It is found that elderly diabetic patients in this community have different degrees of problems in self-cognition of diabetes,selfhealth management ability and related behaviors,and psychological adjustment ability.On the basis of research and interviews,16 elderly people with poor self-health management skills,low self-efficacy,and insufficient blood glucose stability were recruited from 50 patients as service objects for group work,and groups were set up for them.After the group,by comparing the scores of the elderly on the diabetes self-management behavior scale and the self-management efficiency scale before and after the group,combined with the interviews and surveys during and after the group work,quantitative and qualitative evaluations of the service targets were carried out.The results showed that after the end of the group,the client had a significant improvement in diabetes self-health management ability.Therefore,the conclusion of the study is that it is effective to use the method of group work,guided by the theory of self-efficacy,to intervene in the self-health management of elderly diabetic patients through the self-efficacy-enhancing group model. |
