As one of the chronic diseases threatening human health,diabetes can cause a variety of serious complications.Due to the difficulty of cure,patients with diabetes need to struggle with it for a long time and face many physical and psychological burdens.In addition to long-term drug treatment,the most effective way to control the disease is to improve patients’ self-management ability.With the improvement of living standards,the incidence of diabetes is also on the rise,especially in rural areas.However,due to the limited educational level and knowledge ability,many elderly people with diabetes have misunderstandings about the disease,and it is difficult to change the long-term bad behavior habits,so that they ignore the self-management of diabetes,resulting in inadequate self-management ability and low efficiency.Therefore,it is necessary to pay attention to this problem and take relevant measures continuously to improve the self-management ability of elderly diabetic patients in rural areas.This paper aims to improve the self-management ability of elderly diabetic patients in rural areas.Under the guidance of social support theory and cognitive behavioral theory,this paper uses the reciprocity model of group work to design and carry out practical intervention activities.First,taking H village as the survey site,contacted the local health center and medical staff,used questionnaires and interview Outlines to understand the situation of elderly patients,and then used relevant scales to further investigate and analyze the problems and causes.Secondly,14 elderly people with diabetes who were willing and qualified were recruited and randomly divided into control group and intervention group.A reciprocal support group for diabetes was established,and resources of health centers were linked to guide members to improve diabetes cognition,improve bad diet and exercise habits,learn correct ways of blood glucose monitoring and self-care,and improve psychological and emotional adjustment ability.Then in the activity assessment stage,after the comparison of the pre and post test between the two groups,the self-management ability of the intervention group was improved,and the conclusion was drawn: It is appropriate and effective to use the reciprocity model of group work to form a reciprocity support group to intervene in the self-management of elderly diabetic patients in rural areas.On this basis,countermeasures and suggestions are proposed to strengthen the publicity of self-management,give full play to the professional advantages of social work,integrate social resources and improve the service system.The innovation points of the article are mainly embodied as follows: focusing on the object of service and mining its subjectivity.First of all,different from paying attention to the whole patient,this article focuses on elderly diabetic patients in rural areas,and combines the specific reality and objective conditions in rural areas to carry out practical intervention for elderly diabetic patients.Secondly,explore the subjective initiative of the elderly with diabetes in rural areas,respect their subject status,and improve their self-management ability in various aspects through mutual support groups.The shortcomings of this paper are as follows: due to the differences in physical conditions,cognition and other factors,elderly people with diabetes have certain differences in individual activity participation and effect improvement.In the future,in-depth research will be carried out in order to make up for the research deficiencies by combining results from other fields.
