At present,the aging of the society is becoming a big issue and the problem of the chronic diseases of the elderly is getting more and more serious.These diseases have a protracted course and cannot be cured,which greatly influences the lives of the elderly people.Improving the self-management ability of elderly patients with chronic diseases can help improve the life quality of elderly people and promote social harmony and stability.With the development of social work,it has also played an increasingly important role in social services,but in the field of social work,the research on self-management of patients with chronic diseases are still not enough.In this research,the elderly diabetic patients in K community are selected as the research objects.The research is conducted mainly through interview and observation under the theory of ecological system and self-efficacy.The study analyzes the status and problems of improving the self-management capability of the elderly with chronic disease,with the intervention of social work methods.Then it evaluates the effectiveness of the intervention and summarizes the experience.Research shows that the elderly patients with chronic diseases are in poor self-management status,and social work intervention is necessary and feasible.In the process of social work intervention,social workers help them improve their self-management ability from several aspects including the group work which mainly carries out healthy education,psychological adjustment,behavioral change and other activities,supplemented by the case work of improving serious negative emotions,building a community-centered support network,and the self-management ability of the elderly patients with chronic diseases is improved effectively.At the end of this research,it puts forward countermeasures and suggestions for the current problems in the process of promoting self-management of elderly patients with chronic diseases,and reflects on the deficiencies in this study.
