| With the increase of the life span of the elderly group,the human body function gradually decreases,and the types of diseases also gradually increase.Diabetes,as a chronic and progressive non-infectious metabolic disease,has a very high incidence among the elderly.It is also one of the important chronic diseases affecting human health.It will lead to more than 100 kinds of complications and require patients to fight against diseases for a long time.Therefore,diabetic patients need to take medicine for a long time to maintain the normal operation of the body function,which means that diabetic patients must conduct long-term self-health management to control the condition.This study relies on Z Community Nursing home to carry out practical interventional research.Collecting data through questionnaire survey and interview methods,analyzing the current situation of elderly diabetes patients in diabetes self-management in Z community,Longkou City,Shandong Province,Three problems are summarized: insufficient disease cognition and unreasonable self-cognition;lack of management skills and insufficient behavior management ability;high psychological pressure and poor negative emotion management.Based on the current situation of elderly diabetes patients in Z community,the needs of elderly diabetes patients in diabetes self-management in Z community were estimated from the perspective of social work,The need for cognitive change and remodeling,the need for disease management skills improvement and behavior change,the need to alleviate negative emotions,the research around these problems and needs,based on and guided by the theory of cognitive behavior and group dynamics theory,designed and carried out 7group activities in four stages,They are the stage of finding and changing bad cognition and reshaping reasonable belief;mastering disease management skills and correcting external behavior stage;improving negative emotions,realizing the harmonious coexistence of cognition,behavior and emotion;consolidating and strengthening,and maintaining the progress stage.In a qualitative and quantitative manner,the effectiveness of team working model was assessed in self-health management of elderly diabetic patients in Z community.Finally,the whole practical process is reflected from the perspective of theory,practical method and the role of social workers to analyze the problems and deficiencies,Finally,the suggestions of social work to improve the self-management ability of elderly diabetic patients: enrich the intervention forms of social work;improve the professional quality of social workers;integrate resources and multi-linkage;Accurately grasp the stage content and characteristics of the group work.Finally,effective methods and paths for social work to improve the self-management ability of elderly diabetic patients. |
