Effect Of Yaotongkang Decoction Combined With Percutaneous Vertebroplasty In The Treatment Of Osteoporotic Vertebral Compression Fractures On IL-6 & TNF-α

Objective:To investigate the effects of adding yaotongkang decoction to conventional percutaneous vertebroplasty(PVP)treatment for osteoporotic vertebral compression fractures(OVCF)on interleukin-6(IL-6)and tumour necrosis factor(TNF-α),a cytokine indicator of bone metabolism,and to provide a basis for the clinical treatment of OVCF.Method:From October 2021 to August 2022,70 patients with osteoporotic thoracic and lumbar fractures who underwent PVP surgery were selected from the spine surgery department of Qingdao Haici Medical Group(Qingdao Traditional Chinese Medicine Hospital).The patients were divided into two groups using a completely randomized grouping method: Group A(control group)35 cases;Group B(treatment group)35 cases.Control group:regular oral administration of Calcium D,one tablet daily;osteopontin,once daily.Treatment group:On the basis of the control group,Lumbago Kang Tang was given in two doses in the morning and evening after meals,one dose per day.Compare the visual analogue scale(VAS)of pain before and 1 day,1 week,1 month,3 months and 6 months after surgery;bone mineral density(BMD)before and 3 months and 6 months after surgery;IL-6 and TNF-α indexes before and 1 day,3 days,1 week and 1 month after surgery between the two groups,and observe whether patients has any adverse reactions such as allergy,liver and kidney function impairment during the treatment period,and use SPSS25.0 to process the research data and draw conclusions.Result:1.Baseline analysis of observation indicators(1)Before treatment,the general data such as gender,age,height and weight of the two groups were compared and there was no statistical difference(P>0.05),and the data were comparable.(2)Before treatment,the preoperative observation data of VAS score,BMD,IL-6,TNF-α and other indicators of the two groups were compared by statistical analysis,and there was no statistical difference(P>0.05),and the data were comparable.2.Analysis of efficacy index results(1)After treatment,the VAS score of both groups decreased significantly compared with that before surgery,and the range of decline was greater in the treatment group than in the control group;there was no statistical difference between the VAS scores at 1 day,1 month,3months and 6 months after surgery(P>0.05);the VAS scores at 1 week after surgery were significantly lower in the treatment group than in the control group,with statistical difference(P<0.05).(2)After treatment,the BMD value of both groups increased significantly compared with that before surgery,and the rising range of the treatment group was greater than in the control group;there was no statistical difference in the BMD values at 3 months after surgery(P>0.05);the BMD values at 6 months after surgery were significantly higher in the treatment group than in the control group,with a statistical difference(P<0.05).(3)After treatment,IL-6 level increased and then decreased in both groups.At 1 day after surgery,IL-6 levels increased in both groups,with no statistical difference(P>0.05);at 3days and 1 week after surgery,IL-6 levels showed a decreasing trend,and the treatment group was significantly lower than the control group,with a statistical difference(P<0.05);at1 month and 6 months after surgery,IL-6 levels continued to decrease,with no statistical difference(P>0.05).(4)After treatment,TNF-α level increased and then decreased in both groups,and there was no statistically significant difference in the comparison of TNF-α levels at 1 day,3 days,1 week,1 month and 3 months after surgery(P>0.05).(5)After treatment,there was a statistical difference in the total effective rate between the two groups(P<0.05),and the effective rate was significantly higher in the treatment group than in the control group,with better therapeutic effect.Conclusion:The clinical efficacy of adding Yaotongkang decoction to conventional PVP treatment for osteoporotic vertebral compression fractures is remarkable,with positive effects on relieving mid-term pain,increasing BMD value and improving IL-6 level.It is worthy of promotion and application.
