Objective: Based on the analysis of the epidemiological data,clinical characteristics and four diagnostic information characteristics of patients with vestibular migraine in vertigo Diagnosis and Treatment The First Clinical Hospital of Jilin Academy of Chinese Medicine the clinical differences between familial cluster vestibular migraine(FCVM)and sporadic vestibular migraine(SVM)were clarified,and the risk factors of familial cluster vestibular migraine were analyzed.To establish a risk prediction model for familial cluster vestibular migraine.Methods: A total of 600 patients with The First Clinical Hospital of Jilin Academy of Chinese Medicine vestibular migraine from January 2021 to December 2022 were collected and divided into FCVM group and SVM group according to the presence of family genetic characteristics.The patient’s demographic data,disease characteristics,predisposition factors,concomitant symptoms,past history,four diagnoses of traditional Chinese medicine(observation,hearing,inquiry and treatment)and other related information were collected.The vertigo medical record data database was established.The FCVM group and SVM group were analyzed by single factor,and the risk factors of patients were initially screened.The risk factors with statistical differences in the results were included in the binary Logistic regression analysis,and the prediction model of familial cluster vestibular migraine was established.Finally,the ROC curve was drawn,and the established risk prediction model for patients with familial cluster vestibular migraine was verified and evaluated.Results:1.Through univariate analysis,the final results of the two groups of patients showed that age of headache onset,flushed face,tender tongue,thin tongue,red tongue color,tinnitus,light top-heavy,hypochondriacal pain,waist and knee pain,fatigue,thin pulse,irritability,anxiety and depression were associated with the occurrence of familial cluster vestibular migraine.2.The two groups of patients were analyzed by multiple factors,and the final results showed that the main risk factors between the FCVM group and the SVM group were age of onset of headache,hypocholic pain,red tongue,tinnitus,lumbar and knee tenderness,fatigue,irritability and irritability.Based on the results of multivariate logistic regression,the risk prediction model of familial cluster vestibular migraine was established.The formula is as follows:FCVM risk prediction model=0.364-0.063 age of onset of headache +0.514* tongue red+1.050* tinnitus +1.252 hypochetus +1.540* lumbar and knee tenderness +1.259* fatigue+0.515* irritability3.the differentiation and consistency test of the established risk prediction model for familial cluster vestibular migraine showed that the areaunder the ROC curve(AUC)was0.791,P < 0.001,95%CI was 0.755-0.827.The sensitivity was 66.7%,and the specificity was78.0%,which proved that the prediction value of the model was high,indicating that the model had a good diagnostic reality.Conclusion:1.The onset age of headache,redness of tongue,tinnitus,hypochetalgia,lumbago and knee weakness,fatigue,impatience and irritability were the main clinical characteristics differences between FCVM patients and SVM patients.2.The prediction model obtained in this study was tested and the ROC curve was drawn to verify that the prediction was well differentiated and consistent,and had good clinical net benefits,providing new research ideas and methods for the diagnosis of familial cluster vestibular migraine.
