Hypertension is the most common cardiovascular disease and is one of the major public health problems worldwide.As of 2016,there are over 270 million people with hypertension in China.However,the current rate of hypertension in China is only 46.5%,and the treatment rate is only 41.1%,and the control rate is low to 13.8%.The particularity of primary hypertension determines that the treatment process is a "protracted war",and it is imperative to control the harm of hypertension through scientific self-management of the disease.Therefore,this study starts from the self-management needs of the elderly patients with essential hypertension.Through practical research,it is of practical significance to explore the methods of self-management through group work.Through questionnaire,interview,literature research and action research,this study analyzed the current situation and needs of the special group of elderly patients with essential hypertension in M community in jizhou district.With the concept of social work,the method of group work was used to provide social work services for elderly patients with essential hypertension,and to evaluate the overall service effect,so as to provide a practical solution and reference for better meeting the multi-level needs of elderly patients with essential hypertension.First of all,this study analyzed the problems in self-management of elderly patients with essential hypertension in M community from three aspects: disease and behavioral management,emotion management and social role management.Then,from the perspective of medical social work,the needs of elderly patients with primary hypertension in M community were evaluated : It is that demand for therapeutic and behavioral management capability,the need to raise demand for negative emotional energy and the need to build a social support network.Based on the main requirements,the physical and mental conditions of the patients above,the group nature,group goal and group plan were determined.In this study,eight group activities were arranged,and the plan was adjusted according to the specific situation of the service object.In the end,the work of the group has been evaluated through the work of this group,and it has identified the effect of the group's approach to self-management in primary hypertensive patients.At the end of the article,three conclusions are made on this practical process.The intervention of group work can help patients improve their self-management ability;The link and utilization of community medical resources are of certain importance;The motivation of the group is helpful to the recovery of social function.At the same time,two reflections on the practical process of the group were made.The activities should be adjusted according to the actual situation;Service objects should be promoted to actively participate in disease self-management.
