Abstract/Summary:
Objective:This experiment was conducted to investigate whether there is a significant superiority of using the Tui Na manipulation in combination with conventional training,to integrate the Tui Na manipulation into conventional treatment to form a unique combination of Chinese and Western medicine,to observe the function of internal rotation of the shoulder and its effect on quality of life in patients with primary frozen shoulder before and after the intervention,to relieve pain,improve joint mobility and improve muscle strength and proprioception.The treatment is designed to relieve pain,improve joint mobility and improve muscle strength and proprioceptive function.It provides a new protocol for the rehabilitation of patients with primary frozen shoulder and improves clinical efficacy.Methods :A total of 60 patients with primary frozen shoulder were selected from January2022 to January 2023 at the Wangjing Hospital of the Chinese Academy of Traditional Chinese Medicine(only one side of the shoulder was involved,including 30 cases on the left side and 30 cases on the right side).The conventional group was treated with conventional rehabilitation therapy for 30 minutes,the manipulation group was treated with traditional Tui Na manipulation for 30 minutes,and the combined group was treated with conventional therapy and traditional Tui Na manipulation method for a total of 30 minutes.All subjects underwent isometric training of the internal rotation muscle group.In this study,the three groups of subjects were measured before treatment and after 6 weeks of treatment on the following five indicators: Visual Analogue Scale/Score(VAS)for pain,joint mobility(ROM),Constant-Murley shoulder score,horizontal internal rotation peak moment,and active and passive proprioceptive assessment of the internal rotator muscle group of the shoulder.sensory assessment.The data were statistically processed using SPSS25.0 statistical software.One-way ANOVA was :1.Comparison of VAS scores before and after treatment: After 6 weeks of treatment,there was a significant difference in VAS scores between the combined group,the conventional group and the manipulation group compared to the pre-treatment group(p<0.05);comparison between groups: there was a significant difference between the combined group compared to the conventional group after the intervention(P<0.05).There was a significant difference in the combined group compared to the manipulation group after the intervention(P<0.05).There was no significant difference in the conventional group compared to the manipulation group(P>0.05).2.Comparison of ROM scores before and after treatment: After 6 weeks of treatment,the range of motion of the joint in internal rotation,external rotation,abduction and forward flexion at the shoulder level in the combined group,the conventional group and the manipulation group were all improved to varying degrees compared to those before treatment,with significant differences(P<0.05).There was a significant difference in the range of motion of the internal and external rotation of the shoulder in the combined group compared to the conventional group and the manipulation group(P<0.05).3.Comparison of Constant-Murley Shoulder Scale scores before and after treatment:After 6 weeks of treatment,there was a significant difference in the Constant-Murley shoulder scores between the combined group,the conventional group and the manipulation group compared to the pre-treatment group(P<0.05);comparison between groups: there was a significant difference in the Constant-Murley shoulder scores between the combined group and the manipulation group after treatment(P<0.05).There was a significant difference in the Constant-Murley shoulder scores in the combined group compared to the manipulation group(P<0.05).There was a significant difference in Constant-Murley shoulder scores after treatment in the combined group compared to the conventional group(P<0.05).There was no significant difference in the Constant-Murley shoulder scores in the conventional group compared to the manipulation group(P>0.05).4.Comparison of the peak moment of the horizontal internal rotation muscle of shoulder before and after treatment: after 6 weeks of intervention,intra-group comparison: compared with before intervention,the peak moment value of the internal rotation muscle of shoulder in the three groups was significantly different from that before intervention(P<0.05);Comparison between groups: The difference between the combined group and the conventional group was significantly different(P<0.05).There was a significant difference between the combined group and the manipulation group(P<0.05).5.Comparison of joint position perception of shoulder joint horizontal internal rotation muscle group before and after treatment:(1)Comparison of active joint position awareness: after 6 weeks of treatment,there was a significant difference in the difference in active motion position in the combined group compared with that before treatment(P<0.05),and there was a significant difference in the difference in active motion position in the conventional and manipulation groups compared with that before treatment(P<0.05);comparison between groups: there was a significant difference in the difference in active motion position in the combined group compared with that in the manipulation group after treatment(P<0.05),and there was no significant difference in the difference in active motion position in the combined group compared with that in the control group after treatment(P>0.05).There was a significant difference in the difference in active motor position after treatment in the combined group compared with the control group(P<0.05),and no significant difference in the difference in active motor position after treatment in the conventional group compared with the manipulation group(P>0.05).(2)Comparison of passive joint position perception: after 6 weeks of treatment,there was a significant difference in the difference in passive motion position between the combined group,the conventional group and the manipulation group compared to the pre-treatment group(P<0.05);comparison between groups: there was a significant difference in the difference in passive motion position between the combined group and the conventional group after treatment(P<0.05),there was a significant difference in the difference in passive motion position between the combined group and the manipulation group after treatment(P<0.05),and there was no significant difference in the difference in passive motion position between the combined group and the manipulation group after treatment(P>0.05).There was no significant difference in the difference in passive motion position between the conventional group and the manipulation group after treatment(P>0.05).Conclusion :1.After 6 weeks of rehabilitation,patients with primary frozen shoulder were able to relieve pain and improve dysfunction with either conventional rehabilitation or tui-na manipulation,but the combination of the two was more effective in relieving pain,improving dysfunction and enhancing proprioceptive function of the shoulder than conventional training or tui-na manipulation alone.2.The quality of life of patients with primary frozen shoulder was not high.In this experiment,both conventional training and Tui Na manipulation treatment improved the patients’ quality of life,and the combined training group was superior to the other two groups in terms of the improvement of the patients’ quality of life.3.In primary frozen shoulder patients with reduced shoulder proprioception,deeper stimulation of the proprioceptors can better improve the patient’s proprioceptive function and accelerate the recovery process with conventional rehabilitation.
