The Research Of Liqun Song’s Data Mining And Network Pharmacology Of The Experience Prescrip Tion In Differentation And Treatment Of Diabetic Kidney Disease Phase Ⅲ-Ⅳ

Objective:Collecting the medical records of Professor Song Liqun’s diagnosis and treatment of diabetic kidney disease phase Ⅲ-Ⅳ for data mining and summarize his clinical experience and core experience prescription;At the same time,A network pharmacological and molecular docking technology analysis of the common basic prescritions of Professor Li-qun Song for the treatment of diabetic kidney disease phase Ⅲ-Ⅵ was carried put to explore its mechanism of action.Methods:1.Collected 567 patients with diabetic kidney disease phase Ⅲ-Ⅳ in the nephrology clinic of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine from January 2010 to September 2020 as the research object,diagnosed and treated by the tutor Professor Song Liqun.Data mining of symptoms and prescriptions after standard processing of medical records by use the Traditional Chinese Medicine inheritance computing plat form(V3.0).Study and read related medical records and works of Professor Song Liqun and analyze data mining result,exploring the diagnosis and treatment experience and core experience prescription.2.Select Professor Song Liqun’s core experience prescriptions for the treatment of diabetic kidney disease phase Ⅲ-Ⅳ and conduct it by network pharmacology-molecular docking research.Use the TCMSP database to retrieve the effective ingredients and targets of Hnarngqi Power,and the targets related to Diabetic Nephropathy were screened with the help of GeneCards,OMIM and other databases.Bu ild a drug regulatory network with Cytoscape,use STRING database to construct a protein interaction network of Huangqi powder in the treatment of diabetic nephropathy,R was used for functional enrichment analysis of GO(gene ontology)and KEGG(KEGG pathway analysis)genes.Antodock Vina was used for molecular docking of active compon ent-target protein.Results:1.General situation:This study finally included 567 first-diagnosis prescriptions of diabetic kidney disease phase Ⅲ-Ⅳ,a total of 2,194 visits,more men(358 cases,63.14%)than women(209 cases,36.86%);After the syndrome differentiation Mr Song divided the main asthenia syndromes into spleen and kidney deficiency syndrome(401 cases,70.72%)and liver,spleen and kidney deficiency syndrome(166 cases,29.27%),and sthenia syndromes is divided into water-dampness syndrome(259 cases,46.67%),blood stasis syndrome(156 cases,27.51%)and non-sthenia syndrome(152 cases,26.82%).2.Symptoms situation:The main symptoms of the 567 cases included in this study were:Sore waist and knees,nausea and indigestion and loss of appetite,clear and long urine,frequent urination at night,abdominal distension and loose stools,mental and physical fatigue,swelling of hands,feet,limbs and face;The main symptoms of spleen and kidney deficiency syndrome are as follows:abdominal distension and loose stools,shortness of breath,and distinclination to talk,nausea and indigestion and loss of appetite,yellow complexion;The main symptoms of liver spleen kidney deficiency syndrome were as follows:dry throat and dry mouth,night sweat,blurred vision,dry stool,vertigo and dizzy,whole body hectic fever,thirsty and drink plenty of water,feverish sensation in the palms and sole,tinnitus and deafness;The main symptoms of water-dampness syndrome are as follows:swelling of hands,feet,heavy sensation in the limbs and body,obesity,Chest tightness and shortness of breath;The main symptoms of blood stasis syndrome are as follows:sub cutaneous ecchymo sis,ips dark purple,localized tingling and aggravation at night,scaly dry skin,numbness of limbs and hematuria.3.Prescription medication situation:There are 567 prescriptions involving 217 traditional Chinese medicines,the total frequency of use is 9831.The top 10 drugs in frequency are Huangqi,Baizhu,Jinyingzi,Nvzhenzi,Qianshi,Fuling,Fupenzi,Shayuanzi,Tusizi and Baiguo.The characteristic medication of spleen and kidney deficiency syndrome is as follows:Chenpi,Suoyang,Shilianzi and Sangpiaoxiao.The characteristic medication of liver spleen kidney deficiency syndrome is as follows:Sangshen,Mohanlian,Shanzhuyu,Niuxi,Shanzha and Jineijin.The characteristic medication of water dampness syndrome is as follows:Sangbaipi,Lulutong,Chuanshanlong and Sumu.The characteristic medication of blood stasis syndrome is as follows:Raw Rehmannia charcoal,Prepared rehmannia glutinosa charcoal,Circium japonicum charcoal,field thistle charcoal:Danshen,Yujin,Jixueteng and Gualou.The main herbal medicines properties are mild,followed by warm and cold.The taste of the herbal medicines is mainly sweet,bitter and acid.The main meridian tropism of herbal medicines is kidney,spleen and liver.6 pairs of common herbal medicines pairs,9 groups of herbal medicines combinations used by Professor Song Liqun.Cluster analysis showed that 567 prescriptions were clustered into 4 groups.4.Results of Network pharmacology-molecular docking research:After screening,46 active ingredients in Professor Song’s empirical formula can act on 223 targets.of diabetic nephropathy,The main active components were as follows:beta-carotene,beta-sitosterol,ellagicacid,formononetin,isorhamnetin,kaempferol,luteolin,matrine,quercetin,sesamin,etc.The core targets are MYC,JUN,CCND1,TP53,CDKN1A,RBI,AKT1,MAPK1,RELA,CTNNB1,etc.Analysis results of GO function enrichm ent were Activation of polymerase Ⅱ transcription regulatory complex,cyclin dependent protein kinase holoenzyme complex,transcriptional regulatory complex and other biological processes.The results of KEGG pathway enrichment were AGE-RAGE signaling pathway in diabetic complications,human cytomegalovirus infection signaling pathway,insulin resistance,IL-17 signaling pathway,TNF signaling pathway,PI3K Akt signaling pathway,P53 signaling pathway,etc.Molecular docking of active component ligands with receptors,19 pairs of ligand receptor docking results were obtained(Binding free energy≤-5.0KJ·mol-1).Conclusion:1.Huangqi,Baizhu,Fuling,Fupenzi,Nvzhenzi,Qianshi,Jinyingzi and shayuanzi constitute the core experience prescription of P rofessor Song Liqun in treating DKD stage Ⅲ-Ⅳ.2.Profess or Song Liqun thinks that the pathogenesis of diabetic nephropathy stage Ⅲ-Ⅳ is "deficiency of origin and excess of standard".The treatment always adheres to the diagnosis and treatment idea of"Simultaneous regulate of liver,spleen and kidney,tonifying deficiency and strengthening;Simultaneous treatment of dampness and blood stasis,reducing excess and eliminating pathogenic."3.The results of network pharmacology molecular docking showed th at the empirical formula was mainly regulate ages rage,IL-17,TNF and other signaling pathways and participate in inflammatory response,oxidative stress response,glucose and lipid metabolism response,insulin effect,cell growth factor and other biological functions acting on DKD,in order to control the occurrence and development of DKD.
