Analysis Of Professor Xie Liping’s Prescription And Clinical Experience In Treating CKD3-4 Patients Based On The Traditional Chinese Medicine Inheritance Computing Platform

Objective: Through the collection and collation of the medical case data of Professor Xie Liping’s treatment of CKD3-4 patients,retrospective research on them,and with the help of the TCM inheritance computing platform V3.0,the prescription rules and clinical experience of Professor Xie Liping in the treatment of CKD3-4 patients were summarized and discussed.Methods: Select patients with CKD3-4 who met the requirements for admission by Professor Xie Liping in the outpatient clinic of the First Affiliated Hospital of Guangxi University of Chinese Medicine(Dongge Campus)from January2020 to December 2022,and query and collect the patient’s name,gender,age,symptoms,Chinese medicine prescription and disease symptoms and other information through the outpatient medical record system,and the medical case data obtained that meets the requirements is imported into the TCMICS V3.0after information standardization processing,and then through "data statistics" and "prescription analysis" in the software "Symptom analysis" and other functions were statistically analyzed,and the medication rules and clinical experience of Professor Xie Liping in the treatment of patients with CKD3-4were summarized.Results: In this study,a total of 155 effective medical cases of patients with CKD3-4 were collected,and a total of 164 flavors of Chinese medicine were included,with a total of 1917 frequencies,a total of 38 symptoms,a total of 241 frequencies,and a total of 9 types of TCM certificates.In terms of symptom statistical results,"fatigue,poor sleep,low back pain,backache,lower limb edema,frequent nocturia,chills,dark complexion,foamy urine and joint pain" were the top ten symptoms in frequency,of which the symptoms with a frequency of ≥30 included "fatigue,poor sleep,low back pain,back pain,lower limb edema and frequent nocturia",accounting for 57.02%,which was the most common clinical symptom in patients with CKD3-4 stage;In terms of the total results of TCM certificate type,spleen and kidney qi deficiency was the most,with a total of 72 frequencies(accounting for 46.15%),followed by spleen and kidney yang deficiency evidence,a total of 27 frequencies(accounting for 17.31%),the third place was spleen and kidney qi deficiency and wet evidence,a total of 21 times(accounting for 13.46%),the above three types are the most commonly witnessed clinical types in patients with CKD3-4 stage;In terms of the four properties,five tastes,menstruation and efficacy of drugs,warm drugs appeared the most(accounting for 43.24%),flat drugs followed(33.87%),sweet drugs appeared the most frequently(accounting for44.62%),bitter and spicy drugs followed(accounting for 25.22% and 22.22%respectively)in the statistics of five properties,and drugs with the most drugs entering the spleen(accounting for 23.82%),and entering the lungs,kidneys,liver,Stomach and heart meridians also accounted for a large proportion(68.41%in total),drug efficacy statistics were the most effective drugs(accounting for38.07%),followed by water-relieving drugs(accounting for 13.95%),and antitable drugs and blood circulation and stasis removal drugs also accounted for a part of the proportion(8.02% and 7.92%,respectively).In terms of drug frequency statistics,drugs with a frequency ≥ 47 times included Baishu,Poria,Codonopsis,Licorice,Astragalus,Guizhi,Coix Kernel,Angelica,Ox Knee and Eucommia,accounting for a total of 37.61%,which was the core drug used by Professor Xie Liping for the treatment of CKD3-4 patients;In the analysis statistics of drug association rules and core formulas,it was concluded that 9 pairs of drugs commonly used by Professor Xie Liping in the treatment of patients with CKD3-4 were obtained:(1)Baishu,Poria,(2)Poria,Codonopsis(3)Codonopsis,Astragalus(4)Baishu,Codonopsis(5)Baishu,Licorice(6)Poria,Astragalus(7)Licorice,Laurel(8)Codonopsis,Licorice(9)Baishu,Astragalus,etc.(frequency≥46 times),A total of 5 groups of highly related drugs were obtained:(1)Corn whiskers → Poria(2)Coix seeds,corn whiskers → Poria(3)Codonopsis,Coix kernels,Angelica→Astragalus(4)Coix kernels,Angelica→Astragalus(5)Poria,Du Zhong → Baishu,A total of 6 core prescription combinations are obtained as:(1)Codonopsis-Baishu-Tangerine Peel-Guizhi-Dried Ginger-Skullcap-Chai Hu-Licorice(2)Guizhi-Baishu-Pori-Licorice-Aconite-Ginger-Tangerine Peel-Astragalus(3)Astragalus-Codonopsis-Baishu-Poria-Du Zhong-Continuous-Yam-Golden Cherry Seed(4)Astragalus-Angelica-Codonopsis-Cocolum-Coix Kernel-Ox Knee-Rhubarb-Sanqi(5)Codonopsis-Baishu-Poria-Mai Dong-Du Zhong-Fushen-Jujube-Licorice(6)Guizhi-White Peony-Astragalus-Angelica-Codonopsis-Baishu-Fushen-Licorice.Conclusion:1.Deficiency drugs are the core drug category of Professor Xie Liping for the treatment of CKD3-4 patients,and Baishu,Poria,Codonopsis,Licorice,Astragalus,Guizhi,Coix Seed,Angelica,Oxknee and Eucommia are the most commonly used drugs.2.Sweet,bitter and spicy;Temperature,flatness;Spleen meridian,lung meridian,kidney meridian,liver meridian,gastric meridian and heart meridian are the main tastes and attribution of drugs used by Professor Xie Liping in the treatment of patients with CKD3-4.3.Professor Xie Liping’s clinical experience in treating CKD3-4 patients can be summarized as:(1)Give first place to supporting the healthy qi and taking into account the pathogenic factors;(2)To balance and harmony of five zang-organs,and mainly the spleen.
