The incidence of diabetic arterial occlusion of lower extremities(DAO) goes hand in hand with age-growth and it is an important factor impacting on the living quality and average span of life on patients with DAO.Nowdays,it has been revealed that its pathogenesis is associated with endothelial dysfunction.This article investigates the characteristic changes of endothelium-dependent dilation(EDD) and endothelium-independent dilation(EID) by high resolution ultrasound to find the relationship between syndrome differentiation of TCM and vascular endothelial function in patients with DAO.The levels of vascular tension factors as endothelin-l(ET-l),thromboxaneB2(TXB2), 6-keto-prostaglandinF1a(6-Keto-PGF1a) and nitricoxide(NO) are also be studied.40 patients with DAO were selected as the test group (Among them 20 patients were attributed to the syndrome of blood stasis,and the others were attributed to pathogenic dampness-heat attacking the lower limb according to the syndrome differentiation type of TCM).20 patients with non-vascular complication in diabetes or with arteriosclerosis obliterants(ASO) and 20 healthy people respectively were observed as the control group.The results show that EDD of the branchial artery in patients with DAO was reduced significantly compare to those in control. Vascular contractile factors as ET-1 and TXB2 levels were higher than those in control ,while vascular dilatory factors as NO and 6-Keto-PGF1a were declined significantly than those in control.Linear correlation analysis showed that EDD was correlated positively with the levels of NO, 6-Keto-PGF1a, while the levels of ET-1,TXB2 had negative correlation with EDD.EDD of patients with DAO attributed to the syndrome of blood stagnation was reduced significantly compare to ASO group and DM group.
