| Objective:Irritable bowel syndrome (IBS) is a derangement of bowel function syndrome characterized by abdominal pain with abnormal defecation, without the conspicuous change of morphology and biochemistry. According to the character of dejecture,IBS is divided into constipation-predominant IBS,diarrhea-predominant IBS,admixture IBS and uncertainty IBS.Its causative agent and certain pathogenesy is not completely illuminated. The disharmony network of nerve-immunity-endocrine secretion is advanced cognition of IBS at present. The chief change of morbid physiology is the abnormal-stomach intestine dynamia,the visceral hypersensitivity and the derangement of nerve-endocrine secretion. The gastrointestinal peptide hormones not only play a role as neurotransmitters and hormones, but also are concerned with the neuroimmunological mechanism of IBS.The hormones are divided into two subtypes as excitable and suppressive neurotransmitters, the excitability mainly include 5-hydroxytryptamine(5-HT),acetylcholine(Ach),substance P(SP), and so on, the others are mainly nitric oxide(NO),vasoactive intestinal peptide(VIP),calcitonin gene-related peptide(CGRP), and so on. The change of these hormones in stomach intestine and brain influence the network of nerve-immunity-endocrine secretion, make the axle of brain-intestine derangement, and produce a series of change, for example, the anomal-stomach intestine dynamia and the visceral hypersensitivity.IBS belongs to the category of "abdominal pain","diarrhea","constipation" in traditional Chinese medicine, closely correlated with "diarrhea in large intestine',"qi-stagnant constipaton","diarrhea with pain". Its main pathogenesis is stagnation of liver-Qi with deficiency of the spleen and incoordination between the liver and spleen and it falls ill in intestine. Shiyanyongyao possess eliminating wetness-evil with drugs of fragrant flavour,regulating qi to alleviate pain,invigorating stomach and discarding dyspeptic material.On the clinical research, Shiyanyongyao may treat Gastrointestinal dysfunction enteritis,bacillary dysentery and diarrhea effectually. To discuss the mechanism of action of Shiyanyongyao on IBS treatment. By IBS-D rats model, we observe the effect of Shiyanyongyao on mast cells in mucous membrane of colon, some gut hormones.Method:1.IBS-D rats model were established.2.The body weight and diarrhea conditions were observed, calculate the change rate of body weight and exponent of diarrhea.3.HCT,blood electrolytes were detected4. The level of NO in blood serum was detected, so did 5-HT by fluorescence5.The level of VIP and SP in colon were detected by radioimmunoassay6.The tissue appearance in colon and mast cells in mucous membrane were observed by light microscopeResult:1.IBS-D rats showed obvious diarrhea, and weight decreased. Histomorphology displayed epithelium mucosae arranged regulately. Hyperemia,dropsy inflammation and anabrosis were not observed.2.Shiyanyongyao may obviously degrade the exponent of diarrhea (P<0.05); palliate descending weight of IBS-D rats.3.Shiyanyongyao may correct the derangement of blood electrolytes and improve HCT by checking diarrhea4. Shiyanyongyao may up-regulate the level of NO and 5-HT in blood serum(P<0.01),improve the abnormal-stomach intestine dynamia.5.Meanwhile, Shiyanyongyao may down-regulate VIP expression in colon(P<0.05), participate the accommodation of body fluid metabolism6.Shyanyongyao may markedly decrease MC (P<0.01)and SP (P<0.05) expression in colon mucosae, participate the accommodation of visceral sensation.Conclusion:IBS-D rat which established by high lactose food and constraint stress was an ideal model on IBS research. Its disease incidence was high and it was reproducibility well.It simulates IBS of human well.Shiyanyongyao may effectively check diarrhea. Its educed action by accommodating the dilivery of neurotransmitter and inhibiting the ab-normal generation of MC.Finally, Zhongyao may play an important role on the stomach intestine dynamia and the visceral hypersensitivity, in addition, the effectiveness of shiyanyongyao depends on its dosage on the treatment of IBS-D.
