Objective To determinate whether PTH detecting is moreadvantageous in predicting hypocalcemia after totalthyroidectomy than serum calcium, through detecting PTH andserum calcium, combining the symptoms of hypocalcemia andincidental symptoms on the first day postoperation,so as toprovide evidence for long-term calcium supplement. Methods According to PTH value on the first postoperativeday,104patients after total thyroidectomy, were grouped.Every patient’s serum calcium was detected, the amount ofpatients who developed hypocalcemia and the time for calciumsupplement were written down. Results PTH detecting is more accuracy rate than serumcalcium, and there were26patients whose PTH value was <5pg/ml,among them7ones were found transient hypocalcemia andshowed symptoms,while5of them developed hypocalcemia for along time and needed long-term calcium supplement. The positivepredictive value of group PTH value<5pg/ml was28%,and the accuracy rate was46%,higher than other groups.Besides, thepositive predictive value and the accuracy rate of PTH value<5pg/ml was higher than serum calcium.Condusion PTH detecting is more advantageous indetermining hypocalcemia after total thyroidectomy than serumcalcium, and PTH value<5pg/ml postoperation is a reliable indexin predicting hypocalcemia after total thyroidectomy.
