Objective1. To investigate the awareness rate on the prevention and control knowledge of thePTB (pulmonary tuberculosis) of the patients living in rural area of Liyang City, tounderstand the implementation of our DOTS(directly observed treatment+short coursechemotherapy) strategy, and to provide the scientific evidence for carrying out healtheducation of PTB prevention and control work.2.To compare the management effects for the smear positive PTB patients betweenthe traditional supervisory mode with medical personnel and the supervision mode withfamily members, to further explore an optimized treatment management mode of PTBtreatment.3.To provide a theoretical basis for my city in the future PTB prevention and controland research workMethodsBetween Jan2011and Dec2012, we carried out an investigation on the newlydiagnosed PTB patients with the knowledge of transitions, hospital visiting, treatment,prevention and how to assess to the prevention information. All the patients were atrandom assigned into traditional supervisory mode group with medical personnel (medicalobservation group) and the supervision mode group with family members (familyobservation group) respectively. The patients in two groups all were treated with the samechemotherapies (initial smear positive patient:2HRZE/4H3R3, smear positive patient:3HRZE/6H3R3E3). We followed up all the patients to the end of treatment period andinvestigated on the effectiveness of the treatment under the different supervision modes.Chi-square test was used to compare the difference for the rates or proportions between two groups, significance level was designed as0.05, and all tests were two-sided.Results1.Two hundreds and eleven PTB patients including205new smear positive cases(97.1%), and6retreated smear positive cases (2.9%)were recruited in this study totally.196cases (92.8%) were found having regular medication during whole treatment period.175cases (82.9%) finished the regular follow-up medical examination.187patients werecured clinically (88.6%).199patients can do well getting the medicine regularly.Side-effect of medicine cases account for21.8%of all PTB patients.2.Generally, the six core knowledge awareness for the all PTB patients involved inadministration was48.3%. Specially, the top percents,96.2%,90.5%and88.6%were forthe three questions, respectively.1. which way does the PTB spread out through?2. Whatdoes the PTB’s main syndromes?3. Which medical institutions do the PTB patients assessto be examined and treated by?. For the question of what the main prevention methodsfrom the PTB was scored the lowest rate,51.2%. However, no significant difference wasfound for the general awareness rates between family and medical observation groups(51.9%vs44.8%, χ2=1.983,P>0.05).3.In term of compliance, the rate of regular medicine intake for family observationgroup was significantly higher than medical observation group’s (98.1%vs.87.6%, χ2=8.797, P<0.05). The rates for the regular follow-up medical examination also wereshown the same significant difference (89.6%vs76.2%, χ2=6.726, P<0.05).4.Compared with the medical observation group, the cured rate of PTB patients forfamily observation group was significantly higher (93.4%vs.83.8%, χ2=4.809, P<0.05).Conclusions1.Liyang tuberculosis prevention knowledge of tuberculosis awareness rate is low.2.in general, living in scattered, traffic inconvenience in rural areas, effects ofcombined dual mode is better than the simple medical personnel supervision mode.3.we can significantly improve the cure rate of pulmonary tuberculosis patients byimproving patient compliance and follow up rate.Suggestions1.We should make full use of various effective means to expand the knowledge of TBprevention and control, and further strengthen the publicity and education of knowledge ofTB prevention and control of the rural population, by combining with local characteristics, improve the knowledge of TB prevention and control awareness as the focus of the work.2.We can significantly improve the pulmonary tuberculosis patient compliance andfollow up rate by taking the dual mode combined with family members supervision. It isworth promoting the use of in our region.
