Objective:Knowledge of vaginal microecological changes after total hysterectomy and lactobacilli living bacterium capsule on restoring vaginal micro ecological function value.Methods:Select whole hysterectomy patients 73 cases, randomly divided into experimental group(34 cases) and control group(39 cases).Experimental group: in postoperative 1month to place the vagina lactobacillus living bacterium capsule(1/2 times/day), a total of10 days.Control group: don’t give in clinical treatment.Respectively the whole hysterectomy after 1 week, 1 month, 2 months in vaginal secretions of patients with vagina micro ecological environment evaluation.Comparison of two groups of patients with vaginal p H, bacteria concentration, number of flora diversity, lactobacillus, incidence of vaginitis.Results:1. 1 week after total hysterectomy, vaginal flora densities of bacteria in the 67 cases below level II, II- Ⅲ level 4 cases, Ⅳ level in 2 cases, composition ratio are 91.78%,5.48% and 91.78% respectively;Flora diversity in 54 cases below level II, II levels- Ⅲ 18 cases, Ⅳ in 1 case, composition ratio 73.97%, 24.66% and 1.37% respectively;Vaginal p H< 4.5 0 cases, 1 case was between 4.6 5.4, 5.2-5.8 in 7 cases, p H > 5.9 65 cases, constitute than 0.00%, 1.37%, 5.59% and 89.04% respectively;Number of vagina lactobacillus in 59 cases below level II, II- Ⅲ level 14 cases, Ⅳ level 0 cases, the proportion of 80.82%, 19.18% and 0.00% respectively.2.1 month after total hysterectomy: vaginal flora densities of bacteria in the following10 cases II, II levels- Ⅲ 12 cases, Ⅳ 51 cases, composition ratio are 13.70%, 16.44%and 13.70% respectively;Stay below II flora diversity in 8 cases, 6 patients with grade II-Ⅲ, Ⅳ level 59 cases, composition ratio are 10.96%, 8.22% and 10.96%respectively;Vaginal p H < 4.5 6 cases, 14 cases between 4.6 5.4, between 5.2-5.8-15 cases,p H > 5.9 38 cases, composition ratio are 8.22%, 19.18%, 20.55% and 8.22%respectively;Number of vagina lactobacillus in 37 cases below level II, II- Ⅲ level 35 cases, Ⅳ level 1 case, form than respectively 50.68%, 47.95%, 1.37%, bacterial vaginal disease.3.2 month after total hysterectomy: p H < 4.5, 4.6, 5.4, 5.2, 5.2, p H > 5.9, the experimental group were 8 cases, 17 cases, 6 cases, 3 cases, constitute 23.52%, 50.00%,17.65%, 8.82% than the;Control group respectively in 2 cases, 11 cases, 10 cases, 16 cases,constituent ratio, respectively, 5.13%, 28.21%, 25.64%, 28.21%, the experimental group and control group in the vaginal PH than the difference was statistically significant(X2 =14.506, P < 0.05).Bacteria flora intensity in below level II, II- Ⅲ, Ⅳ: the experimental group, 22 cases, 7 cases, 5 cases constitute ratio are 14.71%, 64.71% and 14.71%respectively;The control group(n = 7), 14 cases, 18 cases, composition ratio are 17.95%,35.90% and 17.95% respectively;The experimental group and control group intensity vaginal flora composition ratio difference was statistically significant(X2= 6.640, P <0.05).Flora diversity in below level II, II- Ⅲ, Ⅳ: group 7 cases, 19 cases, 8 cases,composition ratio are 20.59%, 55.88% and 20.59% respectively, the control bacteria each 4cases, 14 cases, 21 cases, composition ratio are 10.26%, 35.90% and 10.26% respectively,the experimental group and control group of vaginal flora diversity form than the difference was statistically significant(X2= 7.094, P < 0.05).Lactobacillus quantity in below level II, II- Ⅲ, Ⅳ: group 1 case, 29 cases, 4 cases, composition ratio are 2.94%,85.29% and 2.94% respectively, the control group(n = 16), 20 cases, 3 cases, composition ratio 41.03%, 51.28%, 7.69%, the experimental group and control group advantage of vagina lactobacillus than difference was statistically significant(P < 0.05).Conclusion:1.1 week after total hysterectomy vaginal micro ecological environment is fragile.2.1 month after total hysterectomy vaginal PH intensity, flora, the flora diversity are constantly in recovery, but the vagina microbial bacterial clumps, had still not returned to normal.3.Live lactobacillus bacteria capsule to restore vaginal micro ecological environment play a role.
