Objective:Tobacco use is the leading preventable cause of death in the world, smoking has close relationship with many diseases' happening or premature death and socio-economic losses, such as multi-site cancer, chronic diseases, reproductive and developmental disorders. This study analyzes the death epidemic characteristics of lung cancer, stomach cancer, breast cancer and smoking status of Hangzhou in 2013, evaluates the effects caused by smoking on human health and socio-economic losses comprehensively, to provide scientific data and decision-making basis to further perfect and implement effectively of tobacco control policies and measures.Methods:Firstly, estimating smoking-attributable risk of lung cancer, stomach cancer, breast cancer by document analysis and population attributable risk assessment method; Secondly, using burden of disease method (healthy life years lost of disease, the economic burden of disease), data collection method and questionnaire survey to get healthy life lost and economic burden of disease attributable to smoking.Results:1. Population attributable risk value of smoking(PAR):lung cancer (male: 53.029%, female 7.968%);stomach cancer(male 15.940%, femaleO.515%); breast cancer 3.084%.2. Disability adjusted life years lost attributable to smoking:the total DALY loss is 6813.577 years, lung cancer (male 5715.577, female 435.095) years, stomach cancer (male 587.547, female 10.528) years, breast cancer 64.830 years. DALY loss attributed to smoking per one thousand people:lung cancer(male 4.801. female 0.372) years, stomach cancer (male 0.494, female 0.009) years, breast cancer 0.055 years. Among Each age DALY loss distribution,60-group is the maximum (lung cancer 4717.291, stomach cancer 478.752, breast cancer 29.331) years, under the age of 20, all set to 0.3. Economic burden attributable to smoking:the total is 6693.80 ten thousand yuan, lung cancer(male 4943.77, female 415.26) ten thousand yuan, stomach cancer(male 1022.23, female 23.40)ten thousand yuan, breast cancer 289.14 ten thousand yuan. The total direct economic burden is 2097.70 ten thousand yuan, lung cancer (male 1139.96, female 111.34) ten thousand yuan, stomach cancer (male 645.10, female 10.50) ten thousand yuan, breast cancer 190.80 ten thousand yuan. Indirect economic burden:early death losses attributable to smoking and smoking cause cost, lung cancer (male 3803.80, female 303.91) ten thousand yuan.stomach cancer (male 377.13, female 12.90) ten thousand yuan, breast cancer 98.33 ten thousand yuan. Years of productive life loss attributable to smoking:lung cancer (male 519.68, female 46.22) years, stomach cancer(male 60.57, female 2.32) years, breast cancer 14.80 years.Conclusions:1. Population attributable risk:female>male, male:lung cancer> stomach cancer, female:lung cancer> breast cancer> stomach cancer.2. Health life loss of disease attributable to smoking of Hangzhou in 2013(DALY):whether the total DALY loss, or DALY loss per one thousand people,female>male; Male:lung cancer>stomach cancer, female:lung cancer>breast cancer>stomach cancer; close to the national level. According to the age groups from high to low, in the order:60-group> 50-group> 40-group> 30-group> 20-group> 10-group (1-group).3. Economic burden attributable to smoking of Hangzhou in 2013:female> male, lung cancer> stomach cancer> breast cancer, men:lung cancer> stomach cancer, female:lung cancer> breast cancer> stomach cancer; There are differences among the direct and indirect economic burden ratios attributable to smoking of three cancers.
