Objective:Estimation of deaths and disease burden caused by main risk factors in Hubei Province can quantitatively analyse its risk for population health,increase awareness of severity for risk factors,and provide a scientific basis for developing interventions and improving the quality of life.Methods:It can obtain the exposure distributions of risk factors in Hubei Province in 2010 and 2013 from the he monitoring system on chronic disease and related risk factors,combined with relative risk(RR)to calculate the population attributable fraction(PAF)through counterfactual attribution method.Mortality data were acquired from a death surveillance system in Hubei Province.Years of life lost(YLL)rate and years lived with disability(YLD)rate cited in this paper were from the global burden of disease study,combined with number of population to calculate the YLLs and YLDs of various diseases,disability-adjusted life years(DALYs)is the sum of YLLs and YLDs.Then we multiplied the PAF by deaths,YLLs,YLDs and DALYs to estimate the attributable disease burden for risk factors.Results:1.The total number of deaths caused by the 11 risk factors in both sexes in 2010 was 180,223,accounting for 52.59%of all deaths,with 124,468 deaths in males,representing 62.49%of all male deaths,and 55,766 in females,corresponding to 39.41%of all female deaths.The deaths for both sexes,males and females in 2013 were 190,676(53.20%of total deaths),129,938(61.61%of total deaths)and 60,738(40.72%of total deaths),respectively.From 2010 to 2013,the growth rate of mortality rate for both sexes,males and remales were 3.85%,3.24%and 7.30%,respectively.2.The YLLs,YLDs and DALYs attributable to the 11 risk factors in Hubei Province in 2010 were 5450.55 thousands years(56.07%of total YLLs),409.17 thousands years(7.16%of total YLDs)and 5859.72 thousands years(37.97%of total DALYs),respectively.In 2013,the YLLs,YLDs and DALYs caused by the 11 risk factors were 5817.89 thousands years(59.71%of total YLLs),483.91 thousands years(7.97%of total YLDs)and 6301.80 thousands years(39.85%of total DALYs),respectively.Compared with results in 2010,the YLL rate,YLD rate and DALY rate in 2013 increased by 5.35%,16.73%and 7.93%,respectively.The DALY rate for males increased from 14265.71 per 100,000 people to 15396.99 per 100,000 people,with the growth rate of 7.93%,while the DALY rate for females increased from 5985.35 per 100,000 people to 6104.78 per 100,000 people,with the growth rate of 2.00%.3.The top three risk factors for disease burden both in 2010 and 2013 were high blood pressure,smoking and high body-mass index(BMI).In 2013,the deaths attributable to the top three risk factors were 52,778(14.68%of total deaths),45,186(12.57%of total deaths)and 21,668(6.03%of total deaths),respectively,and DALYs caused by the three risk factors were 1651.50 thousands years(10.44%of total DALYs),1282.43 thousands years(8.11%of total DALYs)and 740.77 thousands years(4.68%of total DALYs),respectively.The leading risk factor for DALYs in females was high blood pressure,followed by high BMI and diet low in vegetables.However,the top three risk factors for DALYs in males were smoking,high blood pressure and alcohol use.4.The main causes related to each risk factor were different.In 2013,98.96%of total DALYs caused by high blood pressure were related to cardiovascular diseases,high BMI and diet low in fruits were also related to cardiovascular diseases,with 68.13%,and 89,59%of deaths caused by the disease,respectively.In addition,smoking was a main risk factor for neoplasms and chronic respiratory diseases,with 53.07%and 20.90%of DALYs related to the two diseases,respectively.Injuries were related to 61.86%of DALYs caused by alcohol use.Conclusions:The risk factors caused a large number of deaths and disease burden in Hubei Province,especially the high blood pressure,smoking and high BMI,indicating that these factors are highly harmful to population health,therefore,it's important to reinforce the prevention of risk factors,and decrease the population prevalence rate of risk factors.As most of the disease burden caused by risk factors were related with the non-communicable diseases(NCDs),especially the cardiovascular diseases,controlling of the risk factors can effectively reduce the morbidity and mortality of NCDs,then decrease the disease burden of NCDs.The risk factors caused different amount of disease burden in differen sexes,thus specific health promotion and education efforts should be implemented for different sexes.Additionally,the disease burden among males was greater than that of females,informing that more attention should be paied in the risk factors prevention for males.
