BackgroundInfertility has developed into a global issue. According to the survey data of World Health Organization(WHO), over 50% of all the infertile couples encounter male re productive function defects. Many causes can lead to the consequence of male infertil ity. For the case where the percentage of progressive motility(PR) sperm is <32% wh ile the pathogeny is unidentified, it can be generally called idiopathic asthenospermia.At present, the study on the etiology of idiopathic asthenospermia is still a hot spot a nd difficult point in reproductive medicine. The human seminal plasma contains a var iety of proteases and cytokines, which can regulate sperm motility to a certain extent.Vascula rendothelial growth factor(VEGF) is a secreted protein glycosylation by two molecular size 17-22 KD polypeptide chain disulfide connected to form, the molecule itself has a highly conserved heat and acid stability.VEGF is the body of a multifunctional cytokine, can strongly stimulate endothelial cell proliferation and angiogenesis. VEGF in the human reproductive system, mediated by receptors exert their biological effects, promote angiogenesis in various organs in the reproductive system, and involved in the regulation of vascular permeability.In rat sperm acrosome, tail cervical, middle and primary segments were found expressing VEGF receptor Flt-1 and KDR protein, indicating that the sperm itself is the VEGF target cells. VEGF in human seminal plasma has a certain degree of expression, and when the semen of high levels of VEGF, which will also increase the woman's pregnancy.A disintegrin-like and metalloproteinase with thrombospond in type 1 motifs(ADAMTS1) is a class of widely present in the extra cellular matrix(ECM) in the Zn2 + dependent metalloproteinase secreted, it can be degraded brevican, versican and aggrecan and other proteoglycan molecules to participate in ECM remodeling in humans ovary, uterus and urogenital system were expressed.ADAMTS1 may be inhibition of endothelial proliferation and angiogenesis by blocking VEGF receptor phosphorylation. Meanwhile, ADAMTS1 also activated matrix glycoprotein Thrombospondin(TSP), which have anti-angiogenesis effect.Agarwal et al found that the expression level of seminal plasma Asthenospermia patients ADAMTS1 higher than normal men. S.E.Aydos et al study also pointed out that the number of sperm and vitality are negatively correlated with the expression level ADAMTS1 of seminal plasma.One of the important functions of ADAMTS1 is to inhibit angiogenesis,and currently VEGF is the most powerful cytokine in stimulating angiogenesis.Both of them have important significance to the male reproductive system,however,the correlation between them is still not clear.The study adopted enzymeliked immunosorbent assay method for the detection of seminal plasma VEGF and ADAMTS1 expression levels, respectively and the correlation analysis was performed with semen parameters and the outcomes of assisted reproduction treatment. The aim of this study is to investigate the expression of VEGF and ADAMTS1 in human seminal plasma effect on sperm motility. Material and MethodsThe selected object of study for November 2015- 2016 January during the First Affiliated Hospital of Zhengzhou University Center for reproductive medicine visit first artificial insemination by husband's sperm(AIH) help pregnant treatment in infertile patients. Experimental group for 36 idiopathic of asthenospermia patients and the control group for analysis of 31 semen the normal parameters and previous reproductive history of men, fresh semen was collected, routine semen analysis after centrifugal precipitation, absorb the upper seminal plasma, by enzyme linked immunosorbent assay method for the detection of two groups of patients with seminal plasma VEGF, ADAMTS1 expression levels. Results1. The seminal plasma VEGF expression of the idiopathic asthenozoospermia group and normal control group were respectively:77.51±12.47pg/ml ?147.05±15.26pg/ml.The idiopathic asthenozoospermia group compared with the normal control group, the expression of VEGF in seminal plasma decreased and was statistically significant(P< 0.01).2.The seminal plasma of ADAMTS1 expression of idiopathic asthenozoospermia group and normal control group were respectively: 92.38±16.77ng/ml ? 46.87±13.12ng/ml.The idiopathic asthenozoospermia group compared with the normal control group, in the seminal plasma of ADAMTS1 expression increased and had statistical significance(P < 0.01).3.The levels of VEGF in seminal plasma of patients with idiopathic asthenozoospermia group and normal control group were negatively correlated with the level of ADAMTS1(r=-0.433, P< 0.01).4. The level of VEGF in seminal plasma was positively correlated with sperm progressive motility percentage before sperm optimization(r=0.741?0.629, P< 0.01).5. The level of ADAMTS1 in seminal plasma and sperm progressive motility percentage was negatively correlated before sperm optimization(r=-0.347, P< 0.01).6.The clinical pregnancy rates were 11.11% and 12.90% respectively in the patients with idiopathic weak sperm disease and normal control group, and there was no significant difference between them(P> 0.05).7.The levels of VEGF and ADAMTS1 in seminal plasma showed no correlation with sperm progressive motility percentage after sperm optimization method(P> 0.05).Both of them also had no significantly correlation with clinical pregnancy rate(P> 0.05). Conclusions1. The sperm motility was related to the expression level of VEGF and ADAMTS1 in seminal plasma, and the decrease of sperm motility in patients with idiopathic asthenozoospermia was closely related to the decrease of VEGF or the increase of ADAMTS1.2.There was no significant correlation between the expression of VEGF and ADAMTS1 in seminal plasma and clinical pregnancy outcome.
